We are seeking a highly skilled and dedicated Licensed Physical Therapist to join our team at Heartland Home Health.
Job Summary:The successful candidate will be responsible for administering prescribed skilled physical therapy treatment and training for patients with injuries and functional disabilities.
Key Responsibilities:- Collaborate with physicians and clinical team members to develop and implement therapy treatment programs.
- Administer prescribed skilled physical therapy treatment and training to patients.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Stay up-to-date with industry developments and best practices in physical therapy.
- Master's degree from an accredited Physical Therapy Program.
- PT Licensure in the state of practice.
- Current CPR Certification.
- One year experience in hospital or home health preferred.
