At Anova, LPNs are a critical part of the clinical care team. With their skill and expertise, LPNs assist patients in crisis and help with symptom management, wound care, education, and other critical needs.


Responsibilities + Duties

  • Implementing changes to the plan of care as needed with interdisciplinary collaboration with the support of the hospice medical director and / or the attending physician
  • Documenting and communicating all calls, interventions, care plan changes, physician contacts within the care coordination notes of the agency's EMR in a manner consistent with policy and procedure
  • Documenting accurate assessment of patient status and updating the plan of care utilizing appropriate communication (verbal, electronic, written, email documents and databases)
  • Documenting patient care reflecting nursing interventions, patient response to care, focusing on the patient’s goals. Documentation includes evidence of appropriate patient/significant other teaching, and the understanding of these instructions is noted in the medical record
  • Assisting patients, family members or caregivers with concern and empathy; respect confidentiality and privacy and communicate in a courteous and respectful manner
  • Coordinating services with contracted vendors, outside agencies and interdisciplinary team as needed. Investigate and follow through on orders or requests for service or information
  • Providing bereavement resources to the family as appropriate
  • Reporting concerns to the hospice manager as needed
  • Performing blood draws if required

Qualifications + Skills

  • Valid LPN License to practice in the state of Pennsylvania
  • Current BLS/CPR certification
  • Valid Pennsylvania Driver’s license
  • Reliable transportation
  • Strong organizational and communication skills
  • At least 1 year of experience in Hospice care
  • Previous Wound Care experience is preferred
  • Demonstrates effective communication skills.
  • Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
  • Comfortable with basic computer skills and mobile technology (cell phone, applications, and electronic medical record).
  • Must be highly organized and able to manage multiple responsibilities and work on various assignments simultaneously.
