Position Overview:

A Flagstaff state long-term care facility is seeking a dedicated and compassionate Social Worker to oversee and provide medically related social services. This role is critical in maintaining the highest practicable mental and psycho-social well-being of residents at our skilled nursing facility, which serves the long-term care and rehabilitative needs of Arizona’s Veterans.

As the Social Worker, you will manage a personal caseload ensuring compliance with all federal and state regulations as well as facility policies. You will also supervise a team of social workers, providing guidance and support to ensure that the social services program is executed effectively.

Key Responsibilities:
  • Program Management: Plan, organize, and supervise the social services program, ensuring timely assessments and documentation.

  • Staff Supervision: Oversee and support a team of social workers, conduct performance evaluations, and manage staffing decisions.

  • Discharge Planning: Ensure clinically appropriate discharge plans are communicated effectively to all stakeholders.

  • Compliance: Monitor Medicare notices, ensure compliance with federal law, and manage consults for VA specialist referrals.

  • Resident Advocacy: Work with residents, families, and staff to uphold Resident Rights and manage the Grievance Book.

  • Budget Management: Assist in department budget projections, monitor expenditures, and manage staff overtime.

  • Caseload Management: Maintain a personal caseload of Veterans/Members, ensuring quality and compliance in care.

  • Policy Development: Review and develop policies to ensure compliance with regulatory standards. Participate in meetings and special projects.

Required Skills:
  • Knowledge of Veterans Administration, CMS, and State regulatory standards.

  • Strong understanding of social services programs, especially in long-term care.

  • Ability to manage psychiatric issues and geriatric disease processes.

  • Proficiency in developing care plans, case management, and program planning.

  • Strong leadership, supervisory, and communication skills.

Qualifications:
  • Experience: Minimum one-year supervisory experience in a healthcare setting, with direct experience in long-term care.

  • Education: Master’s degree in Social Work (LMSW) required, with an active social work license issued by the Arizona Board of Behavioral Health Examiners.

  • Certifications: Ability to obtain and retain a valid AZ DPS Fingerprint Clearance Card.
