POSITION TITLE: Housing Navvigator- Project Longevity

DEPARTMENT: Clinical Services

FLSA STATUS: Non Exempt

PAY GRADE: $22-$26

REPORTS TO: Assistant Director of Clinical Services


GENERAL DESCRIPTION OF DUTIES

The Housing Navigator provides retention services to assist customers with maintaining Housing security. The Housing Navigator conducts assessments of customer needs and coordinates referrals to Community Renewal Team programs and initiatives, as well as to other Community resources. Promotes service integration within clinical services and works diligently with the Project Longevity program staff.

SPECIFIC DUTIES AND RESPONSIBILITIES

ESSENTIAL JOB FUNCTIONS

  • Assists customers with the completion of rent assistance intake packets
  • Collects data and information from customers, as specified from the Funder
  • Conducts a Household Budget Analysis with customer
  • Assures Housing Counseling service is delivered in the customer’s preferred language
  • Provides tenant/landlord mediation as means to negotiate a mutual agreement between all parties that supports the customer’s Housing security
  • Addresses any concerns raised by tenant and/or landlord
  • Maintains strict confidentiality of information received from customers and/or landlords
  • Follows Agency Steps to Success process, including assessment and evaluation of customer’s needs based on the customer’s Outcome Scale Matrix (OSM) review, and the development of an Individual Service Plan (ISP) that incorporates and addresses customer’s need(s)
  • Ensures customer linkage to ongoing Case Management or care coordination, including making referrals for Programs offered within CRT and Community partners
  • Contacts customers on a monthly basis to ascertain the customer’s current needs, and identify the customer’s barriers to self-reliance and Housing security
  • Maintains open communication with rental assistance Funders
  • Ensures data is entered appropriately and accurately into any databases required by the contract/Funders
  • Maintain accurate computer-based and orderly customer paper files in accordance with Program requirements
  • Prepares reports in accordance with Program requirements and Agency needs
  • Answer/Return phone calls, and reply to email communications within specified limits
  • Regular attendance
  • Attend and complete all mandatory trainings
ADDITIONAL JOB FUNCTIONS

  • Schedules and reschedules customer appointments as applicable.
  • Responds to Supervisor, Upper Management, and Program Funders requests for information as needed.
  • Presents flexibility and willingness to work with customers in accordance with mutually agreed scheduling.
  • Performs other duties as assigned.

MINIMUM TRAINING AND EXPERIENCE

Knowledge: Knowledge of Community Action Agencies; environment and customer-base considered a plus. Must have knowledge of property management and financial literacy experience.

Demonstrate skills: The ability to accomplish job responsibilities under minimal supervision is necessary. Presents excellent oral and written Communication skills that demonstrate the ability to communicate with persons of varying racial/ethnic cultures; economic and social status. Demonstrate proficiency in the use of available technology. The ability to apply Time Management skills to prioritize customer situation, and to function in a fast-pace environment is a must.

Bilingual: preferred – Language Spanish

Education: Associate's degree in Human Services, Social Work or related field from an accredited two-year college or university preferred. Work experience may be substituted for a degree.

Minimum Years of Experience: minimum of 3 years related to property management experience, human services or social work

Driving Required: Yes Agency Vehicle: No Employee’s Own Vehicle: Yes, preferred – with

proof of auto liability ins.

Active Driver License in good standing required upon hire and throughout employment

Physical Ability: Tasks involve sedentary to light work, involving some reaching, handling, fingering and/or feeling of objects and materials. On occasion, tasks involve sustained performance of moderately physically demanding work; combination of climbing and balancing, stooping, kneeling, crouching, and crawling, and may involve the lifting, carrying, pushing, and/or pulling of moderately heavy objects and materials (20-50 pounds).

Sensory Requirements: Some tasks require visual perception.

Environmental Factors: Tasks are regularly performed with minimal exposure to adverse environmental conditions and/or disease or pathogenic substances.
