ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Responsible for completion of the Provider Qualifications process
  • Lead contact with County and Office of Developmental Programs for Provider Qualification process
  • Oversee agency staff driver’s licenses, car inspection, registration and insurance information
  • Gather documentation for specialty services (Registered Nurse, Home and Community Habilitation Enhanced)
  • Complete online Provider Qualification application and submit the hard copy to the County Office
  • Responsible for agency Quality Management Plan
  • Lead contact with County and Office of Developmental Programs for all Quality Management /Risk Management issues
  • Develop Quality Management Plan on a three year cycle
  • Monitor and document data collection
  • Analyze trends and remediate any issues of non-compliance
  • Formalize quarterly and plan reviews
  • Responsible for completion of the QA&I process (Provider Monitoring)
  • Lead contact with County and Office of Developmental Programs for all Provider Monitoring related issues
  • Organize Provider Monitoring documentation for annual self-review for AWC and traditional programs
  • Complete annual online monitoring tool
  • Coordinate on-site County reviews every three years
  • Complete Corrective Action Plan if required
  • Complete remediation activities based on Corrective Action Plan
  • Responsible for COVID-19 Response
  • Act as HCSIS Administrator, prepare paperwork for new services through the HCSIS website and send information to the state for enrollment.
  • Supervise agency programs including Agency With Choice, Residential, Life Sharing, In-Home and Community Supports, and Supported Employment
  • Lead the Human Rights Team and take minutes at the quarterly meetings
  • Assist staff in meeting the requirements for Electronic Visit Verification (EVV)
  • Monitor monthly Exclusion lists for ineligible employees and/or vendors
  • Process new referrals to the agency for traditional services
  • Attend meetings including Provider Alliance and Domiciliary Care
  • Assist with tracking service utilization data
  • Maintain agency ODP Policy and Procedure manual
  • Assist all programs with fiscal year ISP roll-over reviews
  • Attend the annual auditing of Mental Health and Base Services performed by the county
  • All other duties as assigned

EDUCATION AND/OR EXPERIENCE:

  • Completed an undergraduate program at an accredited college or university with a Bachelor’s degree in business administration, human services, or a related field and at least one year experience working directly with individuals with intellectual disabilities.
