A DAY IN THE LIFE OF A REGISTERED NURSE CASE MANAGER

As a full-time RN case manager, you play a critical role in the provision and coordination of care for our home health care agency. You are responsible for preventing adverse events from happening as well as optimizing our health service productivity and patient outcomes. Keeping our organization's mission, vision, and values at the forefront of your mind, you facilitate the delivery of top-quality, patient-centered care while going through your daily tasks. Your tasks fluctuate based on your specific client caseload and schedule. Primarily, you provide direct patient care to acute and chronically ill patients in the comfort of their homes.

In addition, you validate patient assessments through explicit documentation. Helping your patients maintain their personal autonomy is one of your top priorities, and you assist them with managing their own time and learning how to adapt to new situations. You are also diligent about using your critical thinking skills to ensure your patient's safety needs are met, and they always feel comfortable around you. Nothing is more important to you than upholding high standards of care by providing your patients with passionate and outstanding service. You enjoy serving in such an important role with our home health care facility, which is why you thrive in this nursing position


QUALIFICATIONS FOR A REGISTERED NURSE CASE MANAGER

  • Current RN license

WORK SCHEDULE FOR A REGISTERED NURSE CASE MANAGER

This nursing position in home health care works 8 AM - 5 PM, Monday to Friday and on-call at least 1 weekend a month.
