Core Duties:

  • Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in relation to clinical care, quality, and cost-effectiveness.
  • Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF.
  • Perform physical exam and health histories.
  • Provide health promotions, counseling, and education.
  • Administer medications, wound care, and numerous other personalized interventions.
  • Direct and supervise care provide by other healthcare professionals.
  • Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel.
  • Recognize adverse signs and symptoms and quickly react in emergency situations.
  • Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.
  • Make referral appointments and arrange specialty care as appropriate.
  • Perform nursing services identified in the TO.
  • Conduct research in support of improved practice and patient outcomes.

HCW duties in addition to Core Duties:

  • Perform a full range of RN duties in accordance with assignment under the task order, including: Assessment, Planning, Interventions and Evaluations as outlined in National Case Management Standards of Care.
  • Provide comprehensive, holistic, and compassionate medical management services to beneficiaries with catastrophic, serious, chronic, and/or high-risk illnesses or conditions and/or high-utilizer beneficiaries.
  • Participate in a multidisciplinary team with other health care providers providing
  • consultative services pursuant to optimal health care delivery to patients. Including but, not limited RN Case Manager Grand Rounds.
  • Support patient self-management of patient care with external healthcare organizations and facilities, including hospital admission and discharge and referrals from primary care provider to specialty care providers and services in the medical neighborhood to include base resources. Participation shall include, but not be limited to, documentation, and reporting. For cases involving domestic abuse refer the beneficiary to Family Advocacy Program.
  • Manage high risk patient care, including management of patients with multiple comorbidities or high-risk readmission to a hospital setting and recurrent emergency department use to include use of registries and management of high utilizer, polypharmacy, and chronic pain patients. Assure the client is able to safely navigate through the MTF and civilian healthcare systems with no untoward effects.
  • Assist patients with care coordination to include; however, not limited to assisting with referral issues, coordinating care with other clinical care areas within or external to the MTF and communicating with the patient using secure messaging.
  • Perform timely, accurate, and concise documentation of patient admission documentation, within 24-48 hours of intake and individualized case management care plan within 72 hours of intake during regular business hours.
  • Participate in an on-site designated Extension for Community Healthcare Outcomes (ECHO) tele-mentoring training program.
  • Participate in tele-mentoring, tele-consults, and tele-health to include care coordination/training.

REQUIREMENTS:

  • Possess a Baccalaureate Degree in Nursing, a Graduate Diploma in Nursing, or an Associate Degree in Nursing, from an accredited agency such as the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
  • Possess one (1) year of clinical work experience as a full-time RNCM within the preceding three (3) years working in a medical facility.
  • Possess and maintain a current unrestricted license to practice as a registered nurse in any one (1) of the 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.
  • Provide two (2) letters of recommendation written within the last two (2) years attesting to the HCW's clinical skills and abilities. A minimum of one (1) letter must be from a supervisor. Recommendation letters must include name, title, phone number, date of recommendation, address and signature of the individual providing recommendation.
