  • This is a full-time day shift position located at our Dayton Hospice House IPU. (No weekend or holidays)
  • M-F 8am-5pm or 4/10hr days (will need to be flexible to meet the needs of the units)

Job summary:

Accurately processes data and information for the transfer/discharge of a patient in the Hospice House.

Qualifications:

RN's - Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred. Active Registered Nurse license in the State of Ohio without any board actions. Two years acute care nursing experience preferred.

LPN's - Registered LPN with the State of Ohio without any board actions. One-year nursing experience preferred.

  • CPR Certified
  • Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
  • Computer skills sufficient to properly document services and care.
  • Ability to drive during daytime, nighttime or inclement weather.
  • Valid Driver's License with Safe Driving Record
  • State Minimum Automobile Insurance Coverage

Key Responsibilities:

  • Communicates with Medical Director and staff to collect accurate data for scheduled discharges/transfer to and from the Hospice House.
  • Collaborates with caregivers/patients to determine needs at time of transfer to ensure continuity of care at appropriate transfer location.
  • Communicates with caregivers/patients all medication information, including purpose of medication, delivery and order information, and ensures patient’s understanding of medication provided.
  • Collaborates with Medical Director to ensure that all prescriptions are written and available to patient at time of transfer and delivers prescription orders to appropriate destinations and communicates with all appropriate pharmacies to establish needs of the patient and prompt delivery of medications.
  • Orders all necessary DME for transfer/discharge location in collaboration with nurse and social worker.
  • Documents physicians’ orders when received in electronic health record to provide accurate information and ensure safe delivery of services.
  • Assists in other activities and departments when requested to ensure complete and thorough patient care for all Ohio’s Hospice patients.
  • Participates in Continuous Quality Improvement activities to support the quality of Ohio’s Hospice services.
