The Case Assistant reports to the Ryan White Clinical Quality Manager and provides support services for Hidden Garden Medical Case Management as instructed.
Responsibilities:
• Monitor clients’ compliance with primary medical care, mental health, and substance use appointments.
• Provide pre-appointment reminders; assess barriers to keeping appointments and assist with rescheduling HIV primary care, behavioral health and specialty appointments.
• Monitor eligibility for Ryan White services and update agency forms.
• Make phone calls to and on behalf of clients and conduct home visits for lost to care clients.
• Arrange transportation and/or accompany patients to appointments as requested
Qualifications:
• Bachelor’s Degree with two years’ experience working in a social service environment
• Relevant background experience and sensitivity to HIV/AIDS related issues and African-American and other minority community issues.
• An awareness of local AIDS services and resource providers at the public and community levels.
