Community Health Worker

Basic Function:
Works in the community with health care providers, primary care teams, and community based organizations to improve clinical outcomes in a specific target population. Under the supervision
of the State Program Director, assists in the development and delivery of culturally appropriate and community-based education for the specific target population using a health education model for
chronic disease management for management of diabetes mellitus. Collaborates with community based organizations, hospitals, and health care providers to recruit Medicare beneficiaries for
communities or networks for the specific target population to meet project specific quality improvement goals. Collaborates with other assigned team members to provide direct, logistical, and
administrative support for specified recruited communities.
Organizational Relationships:
Reports directly to the QIN-QIO Program Director. Works very closely and supports the Network Task Lead.
Role and Responsibilities:
• Under direction of QIN Program Director, creates and supports intervention communities that seek to meet targeted quality improvement goals.
• In collaboration with Communications Department, other assigned team members and Analytic Services staff, works on assigned project teams to plan, organize, implement, and evaluate
interventions to improve performance on the project measures.
• Under direction of Program Director and the Network Task Lead, recruits potential participants in the assigned intervention communities from a broad array of Medicare beneficiaries
including minority and rural populations that can potentially impact the targeted quality improvement goals.
• Provides one-on-one contacts to intervention community participants, to assist in data collection, follow up to group meetings, and provide intervention tools.
• Coordinates efforts with organizations working toward similar health improvement goals.
• Provides training on chronic disease model with "train the trainer" model to volunteer community health workers recruited in the targeted communities selected for the project.
• Provides education to beneficiaries on the chronic disease management module for management of diabetes mellitus.
• Facilitates communication and coordinate services between providers
• Motivates patients to be an active engaged participant in their health
• Travels extensively to implement/support quality improvement projects. Flexibility in schedule and ability to work some evenings and weekends as needed.
• Takes primary responsibility for all data input and collection required by CMS contract and internal project processes.
• Provides technical assistance to other team members in using and evaluating the effectiveness of specific intervention approaches.
• Other duties as assigned.

Skills/Professional Competencies Required:
• Ability to work collaboratively in team environment, maintaining effective working relationships with all team members, including staff throughout Quality Insights.
• Ability to organize, support, and implement project activities.
• Ability to work with broad array of healthcare providers and community-based organizations, including high-ranking officials and beneficiaries.
• Ability to communicate effectively both orally and in writing
• Ability to work independently, knowing how to seek and use supervision while carrying out a range of complex projects.
• Ability to recognize when to improve needed skills through research, supervision, formal training; has to be a quick and motivated learner.
• Must have operational computer skills, with moderate knowledge of wide range of productivity suite applications and ability to learn new applications in a timely manner.
• Must have ability and willingness to travel throughout state if required up to 75% of time.
Minimum Education/Training Required:
An Associate Degree or higher in a healthcare-related field or Licensed Practical Nurse, preferred.
Minimum Experience Required:
Two years of experience in a health-related field, with preferred experience in ambulatory care setting.
