Salary: $22.55-$26.40/hour

MAJOR DUTIES: The Team Lead works closely with the Vice President of Outpatient and Community-Based Services and is responsible for the provision of services to patients enrolled in the Homeless Outreach and Stabilization Team (HOST) program. This position provides administrative and programmatic oversight and supervision for all counseling and non-counseling staff in the program including service delivery, maintenance of patient records, contract compliance and liaison with various government and private agencies and officials. This position provides clinical services to agency patients. In fulfilling these duties, the incumbent performs the following duties independently:

  1. Support in the ongoing development of the Homeless Outreach, Stabilization and Transition (HOST) Program in Clark County. This program is be designed to serve people who are living with serious substance use disorders or co-occurring substance use and mental health conditions, are experiencing homelessness, and whose severity of behavioral health symptom acuity level creates barriers to accessing and receiving conventional behavioral health services and outreach models. The HOST team works throughout the county to outreach and engage the most vulnerable individuals into services with the ultimate goals of addressing their behavioral and physical health needs, increasing stability, obtaining housing, and transitioning them into long-term services for their substance use disorders or co-occurring substance use disorders and mental health conditions. The HOST team will utilize the principles consistent with modified Assertive Community Treatment (ACT) model that will best serve targeted population.
  2. Monitor service activities and team funcitoning to assure ongoing compliance with the HOST fidelity standards and engagement model.
  1. Support and coordinate the day-to-day operations of the HOST team including scheduling staff work hours to assure appropriate coverage for day, and when necessary, evening, weekend, and holiday shifts and on-call hours; lead the daily organizational staff meetings and treatment planning meetings; continuously work with the clinical supervisor and ARNP/MD to evaluate the status of patients and do appropriate planning and coordination of treatment activities to ensure immediate attention to their changing needs.

  1. Direct and coordinate the HOST participant admission process and treatment, rehabilitation, and support services of the program in coordination with the Clinical Supervisor and/or ARNP/MD. Schedule the outreach contact and screening for eligibility criteria; develop and coordinate the initial assessment and initial service plan; and assign the most appropriate staff to serve the individual as their primary case manager.

  1. Provide direct outreach and case management services to HOST participants.

  1. Utilize the agency’s Electronic Health Record system (Qualifact’s Carelogic) and the Homeless Management Information System (HMIS) to track patient progress and outcomes.

  1. Attend Coordinated Entry meetings and any other community meetings that support coordination with the homeless population.

  1. Review service history of participants to assure no duplication of services with other homeless and behavioral health service providers.

  1. Review Washington Administrative Codes (WACs) and Revised Codes (RCWs) and Federal regulations pertaining to behavioral health treatment and other community support services and develop and implement policies and procedures with the VP of OP and Community-based Services to comply with said codes and regulations;
  2. Support in ensuring that continuous quality improvement addresses both patient needs and compliance with Commission on Accreditation of rehabilitation Facilities (CARF), WACs, RCWs, and Federal Regulations;
  3. Ensure that the agency treatment programs are maintained in such a way to be in substantial compliance with the Department of Health (DOH) and insurance organization reviews, and CARF Surveys;
  4. Prepares and submits statistical reports to the VP of OP and Community-Based Services for program compliance with contract performance indicators;
  5. Provides direct patient care services including individual services planning, care coordination, medication support services, individual and family services.
  6. Provides administrative supervision in collaboration with the Clinical Supervisor of completion of treatment services including case staffing, chart entries, case audits, individual service plan reviews and discharges.
  7. Orientation of all new staff to their assigned duties.
  8. Ensure compliance with all applicable Lifeline Connections contracts;
  9. Prepare and present in-service training to staff in order to meet the needs of the changing trends in the program; and
  10. Other duties as assigned.

POSITION REQUIREMENTS:

  1. A bachelor’s degree or further advanced degree in counseling or one of the social sciences or medical field from an accredited college or university preferred;
  2. Must obtain a WA State Credential to practice prior to starting the position;
  3. Experience directly working with the homeless population required.
  4. Supervisory and program management experience is desirable;

  1. Exhibit Cultural Humility: The continuous application in professional practice of self-reflection and self-critique, learning from patients, and partnership building, with an awareness of the limited ability to understand the patient’s worldview, culture(s), and communities.
  2. Provide Culturally Appropriate Care: Health care services provided with Cultural Humility and an understanding of the patient’s culture and community, and informed by Historical Trauma and the resulting cycle of Adverse Childhood Experiences (ACEs).
  3. Previous documented experience working with an understanding of statistical methods, managing budgets, supervision of personnel and program management, preferred;
  4. Current unencumbered driver license and proof of current car insurance;
  5. Good verbal and written communication skills;
  6. Great attendance; and
  7. Ability to work well with others.

GUIDELINES:

The position requires that the incumbent maintain certification or qualification standards delineated in WAC 246-341, or its successor, and maintains licensure with the Department of Health. The incumbent relies on specialized training and/or equivalent experience in the field of behavioral health treatment, DBHR, WACs and RCWs of Washington, CARF standards and the performance standards developed for the position.

COMPLEXITY:

The incumbent provides program oversight and supervision to patients who differ widely in age and socioeconomic status and who may possess a variety of chronic and serious social, behavioral and psychological problems. Further, they provide administrative supervision for behavioral health counselors, counselor interns, and students. Autonomy, clinical maturity, sound judgment, and creativity are required to help identify and monitor policies and treatment activities that will meet the complex needs of both patients and staff.

PERSONAL CONTACTS:

Contacts are with members of the treatment team, patients, significant others of the patients, Board of Directors, representatives of various community agencies specializing in the treatment of behavioral health and related community representatives including lawyers, probation officers, and other court officials. All personal contacts are carried out in accordance with federal and state laws dealing with the confidentiality of substance use and mental health patient records.

PHYSICAL DEMANDS:

While performing the essential functions of the job, the employee is regularly required to sit, speak clearly, hear and use hands and fingers to manipulate writing utensils and keys on a keyboard. The employee is required to stand, walk, reach with hands or arms, stoop, kneel, crouch or lift and/or move maximum of 15 lbs. The employee is regularly required to effectively operate an automobile.

WORKING ENVIRONMENT:

Working hours are spent in the community meeting with patients in homeless encampments, their homes, health clinics and community agencies. Supervision of outdoor activities, visits to community agencies, and participation in staff retreats and staff development activities will be required.
