Certified Nursing Assistant (CNA)

Shift

40 hours; Monday - Friday 8AM - 4:30PM

Location

Visits patients in the counties of Macon, Jackson, Graham, Swain, Cherokee, and Clay. Travels with mileage reimbursement to nursing homes, patient's homes, and assisted living facilities.

Position Summary:

Provides excellent, professional day-to-day hands-on personal care to individuals with chronic, serious, or life-limiting conditions. Hospice Aides must exhibit competencies required by law and abide by Four Seasons policies in providing personal care to patients, as directed by the written plan of care.

Performs 4 to 7 visits per day depending on location and acuity of patients served. In the event that daily assigned visit numbers exceed this range, additional support will be offered. In the event assigned visit numbers fall below the range, you may be asked to provide support to other team members.

Essential Duties & Responsibilities:
  • Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation
  • As delegated to and directed by the registered nurse:
  • Participates in the coordination of the patient’s hospice care in accordance with professional standards of practice
  • Collects data for ongoing interdisciplinary comprehensive assessments
  • Participates in the development and evaluation of the plan of care by reporting observations attend IDG.
  • Contributes to patient and family counseling and education
  • Perform duties in compliance with Medicare Condition of Participation
  • Practice safety and infection control methods
  • Provision of hands-on personal care

Minimum Qualifications:

Education:

  • High School Diploma/GED

Experience:

  • No more than a 24 month lapse in providing clinical care services upon the completion of a State-Accredited Certified Nursing Assistant course
  • 1 year of clinical experience preferred

Licensures & Certifications: (if applicable)

  • Valid NC CNA certificate
  • Valid driver’s license and proof of automobile insurance
Summary of Physical Requirements:

  • Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling
  • Flexion/Extension of arms/wrist
  • Hand dexterity to include gripping, pushing/pulling and writing
  • Lifting/carrying of 50 lbs. often
