A day in the life of a Registered Nurse Field Supervisor
This RN Case Manager position offers a fantastic schedule The hours are 8:00 am - 4:30 pm Monday through Friday and offers a hybrid schedule working out of your home 3 days a week and in the office 2 days a week.
As an RN Field Supervisor, you get to help your senior/elderly and disabled adult clients stay in their own homes and live with independence and dignity.
Working under the Director of Patient Services, you do more than just provide supervision and coaching for our direct care staff. You help your clients delay the effects of aging and stay as independent as possible by making sure they are mentally stimulated, physically active, and well-nourished. You encourage independence and promote family involvement.
You also ensure that their homes are safe environments, reducing the likelihood of accidents and promoting a timely response to illness. You are meticulous about responding to reports of any changes or concerns promptly. In this way, you provide real peace-of-mind for our clients and their families.
Studies have shown that those who live at home and retain their independence are happier, healthier, and tend to live longer. Your role facilitates this for your senior/elderly and disabled clients through the part you play in our excellent home care services program

QUALIFICATIONS FOR THE RN CASE MANAGER
- Professional Registered Nurse with current unrestricted licensure
- Demonstrate ability to supervise others
- Be knowledgeable regarding community resources
- Be a strong and effective team leader
- Possess excellent communication skills
- Be able to effectively advocate for patients and their families
- Current health exam and immunizations
- Possess a valid New York State Driver's License
- Provide proof of automobile insurance
- Be an effective team leader
