The Social Care Network Director leads and directs a team accountable for executing the day-to-day operations of the Social Care Network and reports directly to the Executive Director. The Social Care Network Director is responsible for achieving Social Care Network goals, establishing priorities, success criteria and managing relationships with key stakeholders including the Department of Health.

Essential Functions:

  • Quickly develops mastery level understanding of the 1115 waiver and related deliverables of the Social Care Network for the Southern Tier of NYS
  • Collaborates across all business divisions to achieve agreed upon deliverables and DOH deadlines associated with the 1115 NYHER waiver.
  • Facilitiates regional deployment of closed loop referral functionality, performance and billing of new Medicaid enhanced services, the successful claims submission process, and timely issuance of payments for adjudicated claims.
  • Coordinate and lead discussions with diverse, cross-functional teams supporting the SCN to make key decisions and remove barriers that hinder the Social Care Network and providers operations.
  • Drive the implementation of internal and external solutions to achieve member satisfaction, cost targets, network adequacy, and efficiency targets.
  • Leads the day-to-day operations which includes the analysis of the business needs and determines how operations should be developed or altered to manage and meet the business needs in the most efficient and effective manner.
  • Evaluates operations to ensure the desired business results are effectively delivered in a manner that complies with corporate policy, regulatory standards and Social Care Network program requirements.
  • Cultivate a culture of high performance, transparency, and collaboration within the organization.
  • Proactively monitors program performance and removes barriers to ensure desired outcomes are achieved.
  • Coordinates with Care Compass Analytics to analyze & model outcomes. Present outcomes with key stakeholders, including providers as part of the contract negotiation process.
  • Monitor provider targets, measure performance (including unit cost, utilization, and quality metrics) and provide feedback and recommendations to executive team and key stakeholders.
  • Speak publicly at conferences or press conferences on behalf of the Social Care Network.
  • Develop and mentor new staff of the Social Care Network and effectively laison with cross functional team members (e.g., Finance, IT, HR, etc.).
  • All other duties as assigned or later identified internally or as published in program documents.

MINIMUM REQUIREMENTS:

  • Education: Bachelor’s degree required
  • 5 years direct supervisor/management experience
  • Excellent written and verbal communication skills that scale to all levels of the organization.
  • High critical thinking skills to evaluate alternatives and present solutions that are consistent with business objectives and strategy.
  • Demonstrated ability to manage people, financial resources, and information.
  • Strong demonstrated ability to establish, implement, and manage effective work processes and organizational structure.
  • Excellent skills in meeting facilitation and interpersonal interactions with the ability to develop strong working relationships (e.g., partnering, conflict management, consulting, etc.)
  • Experience effectively leveraging people, process, and technology to drive and enable desired business results.
  • Skilled with personal computer programs such as Microsoft Outlook, Microsoft Office (Word, Excel, Powerpoint, etc.)

WORK ENVIRONMENT:

Care Compass Network utilizes a hybrid work model where employees are able to work remotely from a home office or from the CCN office, a non-clinical professional office setting. There may be some travel required for partner and/or network meetings.
