Job Summary: This position works to ensure that all students leave Ivy Street with a robust post-secondary plan. This includes supporting families and students through navigation of adult service eligibility. This is a highly collaborative position that works closely with the clinical team, education team, related services team, families and adult service providers.

Job Responsibilities:

Employees in this position perform some or all of the following tasks. Other duties may be assigned.

  • Supports students and their families through the process of securing adult services.
  • Facilitates team (student, family and adult service providers) through the post-secondary planning process (Transition Planning meetings) to ensure a smooth transition and a robust post-secondary plan.
  • Develop and maintain up to date knowledge regarding housing options through adult service agencies and through other community resources.
  • Maintains current knowledge of transition laws, evidence and best practices
  • Maintains spreadsheet of students by age to track adult services and transition needs
  • Follows up with alumni for outcome tracking as structured by CARF guidelines
  • Attends IEP’s for every student 18+ to address transition related needs and outcomes
  • Attends focus meetings of new admissions to gather information related to adult service engagement and post-secondary vision.
  • Attends team every Wednesday for all students 17+ to address and inquire about transition related needs and processes
  • Attends classroom collab meetings for classrooms with students 18+
  • Attends weekly transition team meetings and takes a role in supporting goals related to that team
  • Collaborates with clinical team throughout planning process
  • Contacts adult service agencies as needed for specific students who don’t easily qualify or when there are eligibility questions
    1. Support families when appropriate with appeal process
  • Works with education leadership to ensure 688s are filed and to follow up on 688 status
  • In collaboration with Director of Transition Operations and Chief Transition Officer, supports ongoing development of systems to support transition programming and planning from admission to discharge
  • In collaboration with Director of Transition Operations, networks with outside agencies and organizations to identify resources for students and families related to transition and vocational development
  • Supports programming as requested by supervisor

Other general responsibilities:

  • Maintains First Aid, CPR and CPI certifications
  • Attends ISS monthly trainings

Skill requirements:

  • Bachelor's degree
  • At least 1-2 years of experience working with transition aged youth (14-22) and their families
  • Experience working with state agencies (i.e. DMH, DDS) preferred
  • Excellent verbal and written communication skills
