  • Status: Full-time, 40/hrs. week

What You’ll Do:


The CDDT program serves those who suffer SPMI and from Substance Abuse /Dependence. Our clients have not achieved success in traditional outpatient settings and have struggled to attain sobriety through various attempts at substance treatment. Our team is comprised of a psychiatrist, nurse, therapist, case-manager and peer services. Most services are provided in the community. This is a full-time position with the CDDT team. Case management will include intensive outreach provided almost wholly in the community. Examples of activities include delivering medication, facilitating an educational group, accompanying clients to doctor’s appointments, filling out applications, intervening with landlords, and coordinating care with other service providers, etc.

Duties include, but are not limited to:

  • To carry out rehabilitation and support function and assist in treatment
  • Provide resource acquisition, education, support and crisis intervention under the clinical supervision of staff with professional degrees
  • Update scheduling calendars and general case-management related administrative tasks.
  • Knowledge of diversity and inclusion with the relevant ability to engage, communicate, interact, and work effectively and cooperatively with people of different backgrounds, identities, and culturally diverse communities.

Required:

  • Bachelor's degree in behavioral science
  • 1-3 years minimum of directly relevant experience in the Mental Health Field, Substance Use Disorder field.
  • Experience with mild to severe mental illness and substance use disorders.
  • Strong organizational skills and experience with Microsoft Suite.

Preferred:

  • Experience in coordinating services with other agencies.
  • Bilingual/bi-cultural

The typical hiring range for this position is $24.04 - $28.85/hour, based on relevant years of experience.
