Blended Case Management Program
Program Description:
Blended Case Management (BCM) is a service that assists young people with mental illnesses or emotional disorders by building a strong network of community resources and services. A Case Manager will link individuals with needed services in the community; advocate for children, youth, and young adults and their families; help negotiate complex systems such as special education and social services; assist families in maintaining basic living needs and skills such as employment, housing, food, medical care, and recreation; and provide on-site crisis intervention. The level, frequency, or type of case management service is determined by the intensity of each family’s unique situation.
Position Summary:
The BCM Team Lead is responsible for providing coaching and oversight to BCM staff and managing the coordination of new clients being referred to the Blended Case Management Program while maintaining a BCM caseload. They will engage in marketing efforts and manage and maintain the expected client and staff census.
Required Qualifications:
- A master’s degree in human services or a related field, with 3 or more years of mental health direct care experience
- A bachelor’s degree in human services or a related field, or be a registered nurse, with 5 or more years of mental health direct care experience, 2 of which include supervisory experience
- A bachelor’s degree in nursing and 3 or more years of mental health direct care experience with one or more years of experience working in a Case Management position
- Strong Leadership qualities, including but not limited to time management, work ethic, and passion for service
- Maintain a BCM caseload of up to 20 participants performing all duties specific to a BCM position
- Achieve at least 60 billable units per week
- Supervise On-Call Rotation
- Provide staff training and support staff with case-specific needs
- Work with staff to stay engaged with clients and identify appropriate resources
- Communicate with clients/families who may be disengaged from services and problem-solve engagement issues
- Support proper discharge planning
- Manage incoming referrals and tracking spreadsheets
- Determine client eligibility for program admission
- Assign cases and complete intakes
- Enroll youth into the program’s EMR system and discharge when services are complete
- Provide team coaching
- Ensure elements of service delivery are followed according to program regulation
- Provide additional support and supervision for intensive cases – attend ISPTs, support crises, field complaints, etc.
- Network and coordinate outreach for referrals
- Maintain a working relationship with local school districts and mental health providers enhancing access to care
- Attend stakeholder meetings, as assigned
- Participate in supervision with BCM leadership
- Be available a minimum of 3 evenings per week
- Engage in travel in your own vehicle as required by caseload, supervision, and training
- Transport youth to various community locations
- Use and implement the 5 core values of Trauma-Informed Care in all work practices – Safety, Trustworthiness, Choice, Collaboration, & Empowerment
- Other duties as assigned
- Manage a varied full-time, 40-hour work schedule Monday-Friday between 8:30 AM - 8:30 PM, including at least 3 evenings per week.
- Work in standard office environments, and community settings including client homes, schools, courts, doctor’s offices, and hospitals.
