The Licensed Clinical Social Worker Associate (LCSWA) Clinician provides direct clinical social work services to patients and families to improve and promote overall health, with a focus on addressing the behaviors that impede optimum health.  This role coordinates services with the interdisciplinary team to provide consultation regarding mental health and/or substance use/abuse counseling while facilitating the development of a comprehensive plan of care to address the whole person.  The LCSWA adheres to all internal policies and practices, in accordance with widely accepted professional practice standards, and maintains requirements for professional practice and licensure. 

 

Minimum Qualifications

Knowledgeable in assessment, modalities, treatment planning and counseling techniques and working with comorbid populations (chronic health conditions, mental health, and substance use).  Experience in the use of motivational interviewing, cognitive behavioral therapy, trauma informed care and strengths-based interventions.  Knowledge of and confidence in assessing and diagnosing mental health conditions. Ability to interact with and render services impartially to diverse populations.  Interacts with respect and in a professional manner with patients, staff, external customers and maintains professional boundaries. Must be able to commute independently between CRCHC sites. 

 

·        Experience:  A minimum of 900 field education hours are required.  Preference is given to those who have had field education hours in an integrated care setting.

·        Additional skills required: Knowledge of federal/state regulations and ethical standards for the delivery of clinical social work services and care.  Excellent communication, coaching, and team-oriented skills.  Strong computer skills with knowledge of Microsoft Office and other common office software products.  Able to effectively utilize an electronic health record to document all patient encounters.  Excellent verbal and written communication skills.  Confidence to work independently and utilize supervision effectively in a multidisciplinary team.

  • Additional skills highly preferred: Bilingual in English and Spanish. 

Education: Graduate of a CSWE accredited Master of Social Work (MSW) program.

Certification(s)/Licensure: Current and unrestricted state of North Carolina license as a LCSWA is required.

Key Responsibilities                       

1.      Provide diagnostic and brief therapeutic client-centered interventions, consultations, education, and referral/linkages for individuals with emotional, behavioral, medical, mental health and substance abuse issues.

2.      Collaborate with the interdisciplinary team for coordination of care and implementation of the treatment plan.

3.      Counsel, educate, support, and assist clients and families to gain access to internal and external resources.

4.      Develop and maintain a working knowledge of local and regional external behavioral/mental health, substance abuse and psychiatric resources for clients in need of additional resources.

5.      Participate in Screening Brief Intervention and Referral to Treatment (SBIRT), Medication Assisted Treatment (MAT), Shelter Health and Early Childhood Development (ECD) programs.

6.      Meet or exceed established productivity expectations.

7.      Awareness of personal biases and an ability to deliver culturally sensitive and trauma informed care.

8.      Provide crisis interventions services.

9.      Complete and submit documentation within 72 hours of an encounter.

10.   Participate in individual and peer supervision to ensure utilization of evidence-based treatment plans and maintain necessary workflow.

11.   Provide education and in-service training to staff on topics related to communication and behavioral/mental health topics relevant to the populations served.

12.   Participate in multidisciplinary team meetings, QI meetings and other agency and community-based meetings, as assigned.

13.   Partner with bilingual staff and interpreters to serve Spanish-speaking patients or, if you are professionally proficient bilingual Spanish-speaking, provide support to other staff.

14.   Participate in quality improvement activities by initiating or contributing to monitoring, measuring, analyzing, improving and/or controlling program goals, objectives and/or services.

15.    Support current incentive, regulatory, and certification requirements (such as Grant funded initiatives, Meaningful Use, PCMH, and UDS) through documentation, participation in initiatives, and other activities as directed.

16.   Maintain documentation of all supervision with assigned LCSW supervisor and peer supervision, ensure that supervision meets requirements of NCSWCLB (currently 1:30, supervision/hours worked) and submit documentation to NCSWCLB every 6 months.  Apply for examination eligibility within 2 years of employment.

17.   Perform other duties as assigned.
