JOB TITLE: Care Manager Assistant

REPORTS TO: Program Manager

EFFECTIVE DATE: 06/01/19

EXPECTATION FOR ALL EMPLOYEES:

to work in accordance with System of Care values and principles.

DUTIES:

1. Willingness to attend and complete all CM certification trainings and additional trainings as

assigned by Program Manager

2. As directed by Program Manager, assist Care Managers by making phone calls and documenting

these contacts in Cyber

3. As directed by Program Manager, attend initial meetings with Care Managers when Supervisor

and Team Lead are unavailable

4. Assist Care Managers with filing and documentation upload

5. Attend weekly Team Meetings

6. Attend monthly Staff Meeting

7. BA level CMAs who are interested in applying for a future CM position may be asked to carry a

small caseload (up to 5 youth) at the discretion of the Management Team to help determine their

readiness for promotion to Care Manager.

POSITION QUALIFICATIONS

MINIMUM REQUIREMENTS:

A. Must be enrolled in a Bachelor's degree program in a related field

B. Valid driver’s license and the ability to be covered by the

agency’s vehicle insurance

SKILLS:

A. Basic computer skills

B. Basic understanding of Wraparound model and CSOC
