Job Summary

VNS Health is seeking a skilled Nurse Manager to oversee the day-to-day activities of our specialized programs and products. As a key member of our team, you will be responsible for ensuring the integration of evidence-based care practices into protocols, policies, and quality improvement initiatives.

Key Responsibilities
  • Program Management: Manage the day-to-day activities of one or more of our specialized programs or products, ensuring effective workflows and protocols are in place.
  • Care Coordination: Act as a liaison for care management teams to ensure our programs are meeting expected outcomes, implementing changes as necessary.
  • Clinical Leadership: Assess, educate, and improve patient knowledge of chronic disease, self-care management, and identification of changes in health status.
  • Quality Improvement: Review patient records for readmissions, lead root cause analysis, and develop recommendations for quality improvement measures.
  • Team Management: Supervise staff, conduct team audits, and ensure effective staff training and performance evaluations.
  • Reporting and Analysis: Oversee metric reporting, create weekly departmental KPI reports, and assist senior leadership with client outcomes reporting and data analysis.
Requirements
  • Licensure and Certification: Current registration to practice as a Registered Professional Nurse in NYS, with Care Management or Case Management certification within one year of job entry date.
  • Education: Relevant degree needed for professional licensure, with a Master's Degree in a healthcare-related field preferred.
  • Experience: Minimum of three years of clinical experience, with experience in case management, administration, or discharge planning in a hospital setting preferred.
