Job DescriptionJob DescriptionWe are seeking a compassionate and skilled Registered Nurse to join our hospice care team. The ideal candidate will provide quality end-of-life care and support to our patients and their families. The role involves collaborating with a multidisciplinary team to ensure the physical and emotional well-being of our hospice patients. As an RN in our hospice, you will have the opportunity to make a meaningful difference in the lives of individuals during their most vulnerable moments.

**Responsibilities:**

- Conduct patient assessments and develop individualized care plans.
- Administer medication and treatments as prescribed.
- Monitor patient health and provide necessary interventions.
- Offer emotional support and guidance to patients and their families.
- Coordinate care with physicians, therapists, and other healthcare professionals.
- Maintain accurate and updated medical records.
- Educate patients and families on palliative care and end-of-life processes.

**Qualifications:**

- Valid RN license in CA.
- Minimum 2 years of nursing experience, preferably in hospice or palliative care.
- Strong understanding of pain management and symptom control.
- Excellent communication and interpersonal skills.
- Ability to work effectively within a multidisciplinary team.
- Empathy and sensitivity toward patients and their families.
- Knowledge of hospice regulations and best practices.
