Continuum of Colorado is seeking a Registered Nurse Case Manager to provide oversight of medical services for individuals with intellectual and/or developmental disabilities that we serve in our residential and day program settings throughout Aurora, Denver, and Castle Rock, CO area.


This is a hybrid remote position where the employee will be providing community-based nursing case management services outside of a clinic setting. The Registered Nurse Case Manager is primarily a daytime position with a flexible schedule.


Requirements of the Registered Nurse Case Manager

  • A Registered Nurse (RN) license in the State of Colorado is required
  • A minimum of two (2) years of clinical or home-based nursing preferred; prior experience in developmental disabilities preferred
  • Ability to meet and maintain agency driving requirements and operate agency vehicles
  • Personal vehicle in good operating condition for use during work as appropriate
  • Valid Colorado motor vehicle license
  • Proof of adequate motor vehicle insurance

Additional Responsibilities:

  • Communicate with staff to coordinate medical services to include annual assessments, laboratory appointments, dental care, therapies, vision care, and other follow up as required
  • Serve as a liaison between family, staff, physicians, and pharmacy for the individual’s medical needs
  • Routinely provides health care information and training to staff
  • Assesses and monitors the individuals for appropriate medical services and interventions; implements and facilitates processing of medical orders, including orders that require skilled nursing
  • Provides treatment or telenursing consultation to triage Individuals and/or instructs staff to take the Individual to their physician, urgent care, or hospital for immediate treatment
  • Reviews medical records regularly to ensure the individual’s needs are appropriately addressed
  • Provides after-hours consultation and/or evaluation for urgent and emergency situations on an on-call basis, as assigned
  • Attends and effectively and appropriately participates in the individual’s meetings, med reviews, medical chart audits, clinics, and appointments as needed

More about what you will get to do as our Registered Nurse Case Manager

  • Assures Senior Director and other appropriate agency personnel are well informed of relevant issues.
  • Follows agency policies for Mistreatment, Abuse, Neglect, and Exploitation (MANE)
  • Provides appropriate, effective and respectful communication and interactions regarding medical and other information with individuals, staff, family members, supervisors, agencies and others; maintains individual’s confidentiality.
  • In conjunction with the residential Supervisor, or other authorized personnel, reviews Medication Administration Records (MAR’s) and physician orders monthly to assure accuracy.
  • Completes, reviews and signs off on medical and health-related documentation, as required.
  • Provides medication administration training to agency staff.
  • Provides educational in-services to agency staff in requested medical areas.
  • Trains staff to follow individual Health Care Plans for the individuals.
  • Provides training and observations to staff for Gastrostomy Tube Services.
