SUMMARY
Provide interim case management services for DCFS youth in care through effective care coordination; ensuring timely access and receipt of quality healthcare services.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

  • Conduct a diligent search of historical and current medical documentation.
  • Educate youth in care and caregivers on the HealthWorks Network of services and providers.
  • Ensure timely scheduling of CHE appointment within 7 days or sooner (depending on the benchmark) of speaking to the foster parent/youth in care or group home.
  • Ensure that 90% of assigned cases receive a completed CHE.
  • Closely monitor the Day-15 report and investigate and troubleshoot cases that have reached Day-15 for which no CHE has been scheduled.
  • Ensure that 90% of CHEs are completed within 21 days of the temporary custody.
  • Transport youth in care to appointments by assisting caregivers with 3 or more youth during an appointment or when the caregiver or caseworker is unable to attend the appointment to ensure the CHE appointment is kept within the 21-day timeframe.
  • Ensure 98% of youth in care are enrolled with a primary care physician, preferably with a HealthWorks’ network providers, or a provider of choice prior to case closure.
  • Ensure enrollment of the youth in care with a primary care dentist prior to case closure.
  • Ensure that all necessary documents needed for case closure from the interim process are completed and filed in accordance with HealthWorks standard for file organization.
  • Ensure that cases are prioritized and forwarded to the QI nurse for the completion of the health summary within 48 hours upon receipt of the completed CHE.
  • Ensure timely completion of health services documentation into SACWIS and the in-house database per prescribed timelines.
  • Participate in all required trainings and meetings.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE
Bachelor’s degree and two years related experience and/or training; or equivalent combination of education and experience.

CERTIFICATES, LICENSES, REGISTRATIONS
Valid driver’s license and adequate insurance coverage are essential.
