The Clinical Manager is responsible for the overall planning, direction, coordination, management, and supervision of services provided to patients served by the home hospice clinical team. Participates in the development and achievement of goals and objectives. Participates in staff development functions and assists in hospice educational programming. Provides skilled palliative and end of life nursing care to patients, families, and caregivers in accordance with SSRCH standards, practices, and policies and within the RN scope of practice in WI. This position works collaboratively with the members of the hospice Interdisciplinary Team (IDT) and provides teaching, some direct patient care and supervision of nursing related cares. The Clinical Manager uses the nursing process to assess, make judgments and treatment decisions based on the patients and families wishes identified in the IDT- Plan of Care (POC).

Essential Duties:

  1. Acts as a liaison between Director of Clinical Services and the clinical staff.
  2. Coordination with Intake/Admission RN/Outreach Specialist and Hospice Medical Director on potential admissions.
  3. Ensures all admissions are complete with the regulatory timeframes.
  4. Triages incoming home hospice patient calls.
  5. Oversee schedules for RNCM, Hospice Aides, Team Assistants and Receptionists.
  6. Ensure that Team Assistants are following processes of documentation, sending orders to MD’s, ordering DME and supplies and timely attaching records to the patient EMR.
  7. Participate in the development and achievement of SSRCH’s goals and objectives.
  8. Staff development functions/responsibilities. Provide educational programming and participates in staff orientation.
  9. Participates in new employee and annual training as directed by the Director of Clinical Services and Director of Education and Quality to include Conditions of Participation, HFS 131 and Skills training.
  10. Ensures all staff carries out the POC appropriately by conducting consistent chart reviews.
  11. Ensures completion of Annual Nursing and Hospice Aide Supervisory Visits.
  12. Assists with staff’s performance evaluation and assists and mentors staff with consistent collaboration with Director of Clinical Services and the Education/Quality department.
  13. Collaborate with various department managers to promote positive interdepartmental relationships.
  14. Knowledge of all Electronic Medical Record (EMR) modules for staff training, staff support and, in the development, implementation, and monitoring of an accurate and effective electronic medical record system and provides input into EMR policies and processes.
  15. Knowledge of Medicare Hospice Benefit, philosophy, and specific criteria to establish eligibility for admission to the appropriate level of care. (General Inpatient (GIP), Continuous Care, Routine Care and Respite)
  16. Knowledge of specialized nursing skills for pain and symptom management and end of life care.
  17. Knowledge of county and state regulations for time of death care including coroner case protocols.
  18. Integrates and models excellent standard infection control practice in all aspects of patient and family care and teaching opportunities.
  19. Demonstrates flexibility in work schedules and shared responsibility for delivery of care.
  20. Participates in Manager on Duty Call rotation.
  21. Assist with emergency staffing throughout the organization.
  22. Able to respond to emergency patient situations and communicate staffing concerns with the Director of Clinical Services.
  23. Manages time keeping system for staff.
  24. Manages productivity for RNCM, visit nurses and Hospice Aides.
  25. Participates in Quality Committee.
  26. Collaborates with Education/Quality department regarding needed education and opportunities to improve quality of care.
  27. Understands and works within MVI/SSRCH established guidelines for % of Net Patient Revenue that is allotted to each area.
  28. Promotes the SSRCH educational programs in the community to promote progress, growth and understanding of SSRCH.

Professional Development

  1. Assumes responsibility for own professional growth and development to maintain and improve practice competence.
  2. Develop new skills through continuing education opportunities, nursing journals and other resources.
  3. Able to identify own need for personal support or stress related to working with dying patients and their families and use the SSRCH process to have this need met.
  4. Participate in staff meetings, in-services.
  5. Complete required annual training.
  6. Obtain CHPN certification within 2 years of hire.

Working Conditions

  1. Physical Requirements: Frequent ambulation; constant bending, lifting, twisting, pushing and pulling; reposition patients, lift minimum of 50 pounds unassisted; bilateral manual dexterity; climb up and down stairs; lift up to 350 pounds with assistance; drive to and from patient homes; able to sit for extended periods of time; manual dexterity to operate computer and other office equipment.
  2. Mental Requirements: Read, speak, write and understand English; cognitive processes to interpret patient condition and apply appropriate nursing procedures; ability to add, subtract, multiply, divide and work with fractions; ability to drive and locate patient homes; at times must function independently without supervision while working in collaboration with the IDT.
  3. Working/Environmental Conditions: Fast-paced changing environment, with multiple directives from several disciplines; may at times require work in patient homes with no control over environmental conditions.
  4. Machines, Equipment, Tools Used: IV equipment; glucometer; PT/INR Machine; PCA; AVI; kangaroo pump; computerized equipment; blood pressure equipment; Foley catheter; bed boards; bed cradle; feeding tube; infusion pump; automobile; pager; telephone; oximeter; phototherapy equipment; computer.
  5. Exposures: Reasonably anticipated exposure to blood, other body fluids or other potentially infectious materials. Possible exposure to communicable diseases or infections. Reasonably anticipated exposure to hazardous equipment, assorted chemicals, gases/vapors and other pollutants.
  6. Personal Protective Equipment Required: Assorted gloves; assorted masks, utility gloves; protective eyewear; impervious gowns; impervious aprons; needleless systems as appropriate.

Professional Qualifications

  1. Certificate/Degree/License Requirements: Active WI RN license, valid Wisconsin driver’s license upon hire; re-certification as required.
  2. Experience Requirements: Minimum of 3 years hospice experience (Will consider home health experience). Minimum of 2 years leadership experience (preferred).
  3. Skills Requirements: See Physical and Mental Requirements. Must carry car insurance on vehicle used to perform job. Proficient in Excel, Word and PowerPoint.
  4. Education Requirement: Prefer 4-year bachelor’s degree in nursing or 3-year diploma. Will consider 2-year associate degree with significant experience.
  5. Communicative Skills: Able to communicate directions to RNs, LPNs, CNAs, TAs, and auxiliary personnel; communicates and explains new policies and procedures to agency employees and new hires; able to describe patient condition to physician, patients and family members and offer comfort; able to recognize and respond to non-verbal cues and communication. Can give informational speeches about the SSRCH program to community groups and organizations.
