Summary: The Clinical Case Manager is responsible for providing case management services to individuals who are residing at the Recuperative Care Center. The case manager will engage/re-engage guests, assist in the attainment of maintaining benefits, build relationships with community providers, develop and coordinate the discharge plan, link the individual with ongoing services for post-discharge, remove any perceived or real barriers to service, and actively participate as an integral part of the interdisciplinary team.

Essential Duties & Responsibilities:

  1. Develop and maintain positive relationships with local service agencies, providers, realtors, and community landlords.
  2. Demonstrate knowledge of the principles of growth and development and the ability to apply these principles in assessing and interpreting guest information. Demonstrate working knowledge of substance use disorder, and mental health services within the community and build working relationships with other community organizations.
  3. Demonstrate competency in planning and implementing guest care plans and discharge plans based on age-related needs and input from the individual. Create opportunities and provide support to guests in deciding to develop their skills and foster independence.
  4. Oversee all medical, clinical, and financial needs of the guest during their recuperative stay.
  5. Provide leadership to all staff by creating a positive work climate and creating a cohesive team environment. Communicate with on-call and management staff any concerns or issues that arise related to the care of our guests, staffing concerns, or general needs of the program in the absence of an on-site manager.
  6. Participate in the on-call rotation.
  7. Independently provide case management services to all guests residing in recuperative care. Case management functions include engagement and relationship building, assessment, treatment planning, therapeutic interventions, interdisciplinary coordination, crisis intervention, monitoring referrals, discharge planning, and advocacy.
  8. Conduct individual and group informational/ educational sessions focused on ensuring housing stabilization, housing challenges, and other issues.
  9. Work with guests on maximizing benefits and income including entitlements, work opportunities, and insurance management.
  10. Regularly coordinate and participate in interdisciplinary case conferences with other providers at Recuperative Ace and within the community.
  11. Maintain core training certifications including CPR/F and OSHA.
  12. Attend staff meetings, meetings, and in-service training to maintain applicable core training and certifications.
  13. Other duties as assigned.
  14. Maintained guest medical records in TIER.
  15. Provide support to guests with completing housing applications/ education applications and other needs associated with housing and continuing education documents.

Required Education/Experience:

  1. Preferred educational background: Bachelor's Degree or equivalent license with either the Department of Public Health or Department of Mental Health. A minimum of three years of clinical case management experience with an emphasis on care planning or discharge planning.
  2. Experience managing individuals with substance use disorder and mental health conditions and comfortable working with homeless and/or low-income individuals.
  3. Demonstrate ability to work independently, coordinate documentation flow as required, and interface with management and medical services staff as needed.

Required Skills/Knowledge:

  1. Knowledge of housing services, challenges, and resources within demographics is preferred.
  2. Knowledge of Massachusetts entitlements, housing subsidies, vocational opportunities, and State insurance programs.
  3. Excellent organizational and problem-solving skills.
  4. Warmth, empathy, and ability to set limits while maintaining a positive and respectful attitude. Flexibility, good judgment, organizational skills, and reliable
  5. Proficient in using computers, word processing, spreadsheets, and navigation of the internet.
  6. Bilingual/bicultural preferred especially in Spanish or Khmer.
