Job Summary

Rakhma seeks an upbeat, kind, positive Home Manager to provide exceptional leadership to our staff and residents as part of a dynamic team. The Home Manager will design, deliver and document person-centered outcomes, implement individual care plans, and foster high-quality programming to support resident’s physical and cognitive health. They engage in resident assessments in partnership with medical staff and resident families.

The Home Manager manages and coaches a team of approximately 15 staff in the mission, vision and values of Rakhma Homes. Home Managers also manage site budgets, perform quality assessment reviews (residents/staff), ensure compliance with all State and Federal regulations, communication with families/vendors/volunteers, and provide shadow shifts/training to incoming staff. Home Managers help with marketing and work to build the Rakhma brand. The Home Manager works closely with a Director of Health Services, Human Resources, the Facility and Maintenance Manager, and the Executive Director to deliver quality resident services and maintain the home. The Home Manager reports to the Director of Health Services.


ESSENTIAL DUTIES AND RESPONSIBILITIES


Staff supervision

· Hire, onboard, train, assess, coach, schedule, and supervise direct care staff. Perform regular staff “check-ins” and performance reviews. Work with Human Resources to ensure adherence to all employment laws.

  • Ensure competency of all new hires. Work with Director of Health Services to maintain compliance to training regulations.
  • Implement daily, weekly, and monthly task outlines to warrant safe house operations.
  • Oversee timekeeping of all staff, manage staffing levels, and maintain master schedule for all direct reports.
  • Ensure staff adhere to PPE and other safety expectations, including utilizing proper sterilization and disinfection techniques. Retrain and remediate as deemed necessary.
  • On-call on a rotating basis.

Resident care

· Partner with resident families and care teams to create individualized person-centered services for 10-15 residents. Ensure staff abide by these established protocols.

· Design programming to meet resident needs.

· Participate in resident care conferences to address changes in resident needs. Participate in referral assessments and ongoing care meetings. Review staff reports on resident conditions and work to correct any incidents that occurred, including medication administration errors.

· Support waiver process to indicate changes in resident condition. Adhere to all grant funded reporting requirements.

· Analyze vulnerable adult reports, grievances, and accident/incident reports in a timely manner; investigate, evaluate, and determine remediation and prevention of future risk.

· Conduct new resident tours, in person and remote, with prospective residents and their care teams.

· Implement and maintain communication systems between residents and family members.

Operations

· Work to guarantee license requirements are met.

· Plan, order, and oversee implementation of a nutritious menu with adequate materials for preparation.

  • Oversee facility cleanliness and safety, and regularly confirm that all systems are in working order. Maintain emergency preparedness and response plan through required drills.

· Coordinate garbage and recycling programs, and organization and cleaning protocols for each house. This includes ensuring the appropriate supplies on available for use. Submit workorders for repairs as needed.

· Maintain house budget, ensuring staffing and expenses remain within expected parameters.

· Turn in administrative paperwork to business office (expense receipts and staff schedules).

· Foster an environment of open communication with direct reports and other management staff through attitude, responding to communications promptly in a professional manner, participation in meetings, and providing ongoing collaborative response to company concerns.

  • Prepare for and participate in state survey process and cooperate with MN Dept of Health officials regarding complaint investigations. Write and implement corrective action plans within required timeline of receiving reports.
  • Maintain and perform on-site audits and mock surveys.
  • Prepare for and help facilitate quarterly Safety Committee Meeting.

Communications

· Gain strong knowledge in Rakhma’s policies and procedures, communicate to direct reports, and ensure compliance.

· Communicate proficiently by verbal, written, and electronic means with diverse stakeholders – to include staff, residents, families, medical professionals, vendors, administrative staff, and others.

· Promote the mission, vision, and values of Rakhma in all areas of responsibility.



Requirements:

DESIRED QUALIFICATIONS · Bachelor’s degree in Human Services, Management or related field plus 2 years leadership experience; OR 5 years in a residential group home with leadership and management experience; OR an equivalent combination of education and experience to exceed five years. (Higher wage structure for LALD)· Experience in a residential program serving seniors, people with disabilities or other related models; residential memory care experience preferred.

  • Knowledge of State of MN DHS, Health Home Care Statutes, MN Food Code and HCBS setting rules helpful.
  • Reliable transportation, a valid driver’s license, a good driving record, and proof of vehicle insurance.
  • Computer proficiency to include MS Office, HRIS, Eldermark, other digital platforms.

PHYSICAL DEMANDS· Use of office equipment, to include computer, printer, copier, fax machine, with extended periods of sitting alternating with long periods of time.· Occasional time “on the floor”, as needed, to fulfill needs of the house. Occasional lifting up to 50 lbs. · Walking, bending, stooping, twisting, full range of motion, manual dexterity, and eye-hand coordination.· Be able to take on care giving responsibilities as needed.· Driving between residences throughout Twin Cities area.


WORKING CONDITIONS· Home Managers usually work in office setting within one of the residences with a requirement of 25% of time being spent “on the floor” with residents and Care Partners, not considering On-Call shifts. · Time spent in the community advocating and building awareness of Rakhma.· Driving between residences or completing tasks in the community, as needed.
