General Responsibilities

  1. Conduct comprehensive needs assessments and provide coordination of services to assist identified population to gain access to and receive needed services.
  2. Provide case management services in an office setting or field based setting.
  3. Provide clients with referrals to outside supports as desired and as needed to support the client plan of care.
  4. Determine eligibility for services including state, federal, and local programs.
  5. Establish if indicated, a written care or treatment plan with the client and family to enable the client to live with maximum possible independence.
  6. Provide active intervention and advocacy on behalf of the client to access necessary services from state, federal, and local programs.
  7. Monitor ongoing client services, advocate as needed for assigned client caseload.
  8. Conduct comprehensive assessments bedside at the hospital, nursing home, or client home.
  9. Assist with the development of program reports and present to hospital administration which includes readmission data information; compute readmission data as needed.
  10. Work with caregivers of those clients being served by CCP.

Essential Functions

  1. Support clients in the Community Care Program.
  2. Serve as a back-up leader for Title III Community Based Programming, such as caregiver education and caregiver support.
  3. Handle more complex cases and support clients with multiple medical and socioeconomic issues.
  4. Maintain a cooperative working relationship with service providers and community organizations to meet client needs.
  5. Create and maintain confidential case records. Meet or exceed all program, funding source, and/or Standards of Excellence standards related to case documentation.
  6. Complete all agency, program and other required reporting or documentation in an accurate manner according to required timeframes.
  7. Work independently and as part of a team, bringing concerns or challenges regarding complex issues to the attention of direct supervisor.
  8. Coordinate work schedule for adequate program coverage and utilize agency time and resources effectively.
  9. Act as Agency ambassador in communities served.
  10. Maintain required certifications, including the completions of any recertification or annual training requirements.
  11. Maintain a strong knowledge of available programs available to clients in the Agency’s service area.

Requirements:
  1. A Master’s degree is required with preferences in social work. All degrees must come from a regionally accredited educational institution which has U.S. Department of Education approval.
  2. Minimum of three (3) years of experience providing case management services is required.
  3. Maintain required certifications Community Care Program Certification.
  4. Must possess valid Illinois licensure status (LSW or LCSW).
  5. Experience in program specific population is preferred.
  6. Ability to relate well to individuals with disabilities and older individuals respecting their privacy, dignity, and right to self-determination.
  7. Demonstrated ability to work effectively and constructively with persons from all racial, diverse cultures, family systems, and social backgrounds.
  8. Strong verbal and written communications skills.
  9. Passage of complete background clearance.
  10. A valid driver’s license, reliable means of transportation and proof of liability insurance is required.
