We're looking for someone to join our team as a Population Health Coordinator who:

The Population Health Coordinator (PHC) is primarily responsible for managing the implementation of identified sections of the Quality Department strategic plan with a focus on improving the foundational building blocks for high functioning primary care. The PHC is knowledgeable about workflows for clinical operations and documentation, quality care improvement frameworks, and promoting collaboration between centralized and site-based teams. The focus project for this PHC position is to develop and implement patient empanelment systems and tools to promote continuity of care and patient safety, while also enhancing staff and patient experience when providing that care. The PHC will dedicate approximately 0.4FTE to serve in the role of PHMI empanelment and work closely with the operations and medical departments to manager panel assignments, track reports with key empanelment metrics, and conduct trainings as needed. With consideration to Social Determinants of Health (SDOH), the PHC will display compassion by connecting patients with local resources available to support their holistic health. The PHC will exercise evidence-based care coordination techniques to meet the patient’s health-related needs and preferences while ensuring operational efficiency is maintained. The PHC plays a pivotal role in connecting patients to services at Clinica Sierra Vista and working with the care team.

Essential Functions:

  1. Implement a system for empanelment and other special projects as assigned.
  2. Review data and implement quality improvement strategies related to empanelment and quality.
  3. Serves as a liaison between health and social services and the community to facilitate access to services and improve the cultural competence of services being provided.
  4. Provides qualified data to the Manager of Quality and Population Health for the purpose of developing training programs, policies, and procedures.
  1. Recommend interventions or strategic adjustments to the Manager of Quality and Population Health so the practice may improve clinical quality and operational efficiency.
  2. Collaborate with health centers as assigned to facilitate care team development, Population Health Management Initiative (PHMI), and progress towards improved processes and clinical outcomes.
  3. Utilize quality improvement tools when testing a new system of care including PDSA rapid cycles for change.
  4. Collaborates with Quality, HIT, and operations to facilitate and implement the Population Health Management Plan and empanelment. Participates in quality projects with external partners such as PHMI and health plans.
  5. May serve a quality liaison to campuses as assigned.

You'll be successful with the following qualifications:

  1. Medical Assistant or nursing degree is highly desired.
  2. Certification as a Community Health Worker (CHW) highly desired.
  3. High School Diploma or GED required.
  4. Minimum of 3-4 years of healthcare experience. Preferably in the ambulatory care setting. Working knowledge of HEDIS, UDS, PCMH, and FQHC.
  5. Spanish speaking highly preferred.
  6. Maintain excellent internal and external customer service at all times.
  7. Possess the tact necessary to deal effectively with patients, providers, and employees, while maintaining confidentiality.
  8. Must be able to work independently, multitask, and handle a high volume of work.
  9. Must be reliable with attendance.
  10. Must be highly organized and detail oriented.
