The primary responsibility of this role is to screen individuals for admission into NorthSight Recovery’s outpatient and transitional housing programs and conduct comprehensive assessments of individuals seeking services. Intake Assessment Specialist will have a deep understanding of mental health disorders, substance abuse issues, co-occurring disorders and health related social needs. They will utilize their clinical expertise and required assessment tools to evaluate clients' needs, provide provisional diagnoses, develop initial treatment plans, and coordinate care with the outpatient team. Strong written and verbal communication and interpersonal skills are essential for collaborating with clients, their families, and other healthcare professionals to provide complete and accurate assessments to inform the course of treatment.

Job responsibilities include:

  • Conducting clinical and Transitional Living prescreens, including the evaluation and review of results with clinical and medical leadership as necessary.
  • Conduct comprehensive assessments of individuals with behavioral health concerns, including mental health disorders, substance abuse issues, and co-occurring conditions. ·
  • Providing provisional diagnoses based upon information gathered assessment and historical documents.
  • Collect and analyze relevant information, including medical history, social background, and current symptoms, to form a comprehensive understanding of each client's needs.
  • Utilize standardized assessment tools and clinical interviews to evaluate the severity of symptoms and functional impairment.
  • Participate in the development of the Individual Service Plan.
  • Coordinate care with other healthcare professionals, including psychiatrists, psychologists, social workers, and case managers, to ensure comprehensive assessment and accurate information for treating therapist.
  • Maintain accurate and timely documentation of screening, assessments, initial treatment plans, and progress notes in accordance with regulatory requirements and organizational policies.
  • Participate in staff meetings, and continuing education opportunities to enhance clinical skills and stay updated on best practices in behavioral health assessment and treatment.
  • Adhere to ethical and professional standards of conduct, including maintaining client confidentiality and respecting cultural and individual differences. In concert with clinical leadership, identify evidenced based patient screening tools and/or develop agency specific patient screening tools using evidence based principles.
  • Demonstration of an understanding of and adherence to all company policies and procedures.
  • Other related duties as assigned


Qualifications:

  • Experience working in a co-occurring clinical inpatient or outpatient setting.
  • Experience performing comprehensive assessments co-occurring population; Arizona Medicaid experience preferred Ability to identify relevant clinical and medical issues that have a bearing admission criteria and patient care.
  • Excellent organizational skills and an eye for detail.
  • Strong internal and external customer service skills.
  • Bachelor’s degree preferred, but will consider combination of relevant experience in lieu.
  • Minimum of two years’ experience in behavioral health field. Proficiency in using electronic health record (EHR) systems and other technology tools for documentation and communication.
  • Ability to work independently and as part of a team in a fast-paced clinical environment.
