Job Summary: This position is primarily responsible for providing case management services and coordination of all services needed by Daily Planet consumers, to include medical, mental health, dental, vision, substance abuse, housing, benefits and insurance acquisition, as well as other social service needs. Protects the dignity, privacy and confidentiality of patients and their families; and works collaboratively with other Daily Planet providers and staff to promote a team-based approach to patient care.

Job Requirements: Bachelor’s degree in a field of Science required, Master’s degree in Social Work preferred. One (1) to three (3) years of general clinical case management experience required. Bilingual (English/Spanish) preferred. Knowledge in chemical dependency, mental health and housing and homeless service systems required. Skills include excellent oral and written communications including telephone, organizational, and personal computer including word processing and Microsoft office. Must demonstrate skill and knowledge in case management, client assessment and engagement and group exercises.
