CASES is looking for a dedicated and detail-oriented individual to join our team as a Clinical Case Coordinator in our Intensive Services Unit. This role plays a crucial part in supporting participants' successful community integration and rehabilitation.
Upon a participant's admission to the program, the Clinical Case Coordinator conducts an introductory supervision meeting to outline the supervision plan and explain the stipulated conditions. They guide participants in adhering to supervision appointments and court appearances, ensuring they actively engage with the legal process.
Additionally, the Clinical Case Coordinator performs ongoing assessments to identify service needs and works collaboratively with participants to develop tailored service plans that address their specific goals and needs. By fostering a participant-centered approach, the coordinator helps reduce recidivism and enhances community treatment engagement.
The ideal candidate will demonstrate strong organizational and communication skills, with a proven ability to prioritize tasks effectively. They will work closely with a clinical team and a Peer Specialist to ensure a cohesive and comprehensive approach to participant care. A commitment to the principles of rehabilitation and community reintegration is essential for this role, emphasizing the importance of supporting participants in achieving their self-identified goals for treatment and recovery.

Shift Hours: This position is full-time Tuesday-Saturday 9AM-5PM or 10AM-6PM
Salary: The salary for this role is $67,500 annually

Workplace Flexibility: FlexibleWhat You Will Be Doing:
- Provide supervision and community-based services to a caseload of participants.
- Within 24-48 hours of a participant's release, the successful candidate will conduct Supervision Introduction/Orientation meetings to review supervision conditions, reporting schedules based on recidivism risk levels, and mandatory communication methods (face-to-face, telephone, and text) outlined in the participant's supervision plan.
- Will work closely with the court and other pretrial staff to ensure participants successfully attend mandated supervision appointments and court appearances. They will review legal and court screening documentation, helping participants analyze their offending behavior patterns and identify areas for behavioral change to reduce the likelihood of future arrests.
- Conducting ongoing service need assessments to understand participants' psychosocial needs and problems. This includes assessing the severity of mental health and substance use disorders, identifying barriers to participation and engagement, and evaluating individual motivation for treatment. Link participants to appropriate treatments based on their eligibility, preferences, and needs, even providing escorts to community services if necessary.
- Ensure the timely flow of community outreach and case management activities to meet program goals and objectives.
- Utilize problem solving techniques, motivational interviewing as an integral part of the pretrial supervision and case management process approach, to help motivate and engage individuals in any needed process of change.
- Build and maintain collaborative relationships with community providers, such as supported housing providers, Health Homes, care management programs, addiction treatment centers, mental health providers, peer-run services, education and vocational training providers, legal services, medical care providers, and family services, is essential.
- Clinical Case Coordinator will also address complex issues and follow community-based monitoring protocols, especially for high-risk participants. They will be responsible for entering assessment data, supervision appointment reporting, and progress notes into the program database, ensuring all electronic charting activities are completed accurately and confidentially.
What We Are Looking For:
- Master’s degree Required.
- Minimum of 2-3 years’ experience working in social services with individuals with substance use disorders, co-occurring mental health and involvement in the criminal justice system.
- Knowledge of community treatment, support services and resources.
- Experience understanding family systems based on diversity of cultural and ethnic backgrounds.
- Highly organized and self-motivated.
