POSITION SUMMARY
This position is responsible for the primary case management for Family Treatment Court (FTC) participants.
ESSENTIAL DUTIES
• Ensure participants understand program requirements and schedule for services, and ensure they receive the Participant Handbook and provider contact information
• Develop individual case plans based on participant assessments including FTC requirements, community resources, and personal goals; FTC case plans will complement DFCS case plans, and will be revised as new assessments are conducted
• Maintain regular contact with participants, face to face (in person or virtual) at least weekly, and more often via phone, text and/or email
• Participant check-ins will include:
o Treatment compliance
o Drug screen results
o Surveillance reports
o NA/AA groups, support groups and sponsor contact
o Incentives/sanctions
o Employment/education updates
o Housing concerns
o Child visitation
o Other resources/services, as indicated
• Make referrals to community resources as needed; assist participants with applications/enrollment and appointments
• Research and be knowledgeable on community supports, including employment and education opportunities, housing, in-patient treatment, support groups, childcare, and DHS OFI services such as TANF, CAPS, and Medicaid
• Provide crisis management for participant lapse/relapse and other situations that may disrupt the participant’s progress in FTC; assist with referrals for therapeutic response as needed, including in-patient treatment
• Coordinate treatment groups; maintain communication with providers on participant attendance and engagement
• Coordinate random drug screens with Accountability Court Lab; maintain communication with Lab on participant compliance and test results
• Conduct Phase Up/Graduation appointments with participants to review paperwork and requirements of Phase Up/Graduation packet to present to the FTC team
• Purchase incentives; put together gift bags for phase up and graduation
• Under the direction of the FTC Coordinator, participate in team meetings and prepare updates for court staffing; lead court staffing in the absence of the FTC Coordinator
• Provide support to the FTC Coordinator in preparing grant reports and court notes
• Utilize Case Worx or current case management software for all notes; maintain up to date documentation; maintain participant electronic and paper files
• Update GPRA evaluations in SPARS every six months (SAMHSA grant requirement)
• Serve as liaison for communication between hearings, facilitating communication between team members and sending interim updates
• Attend and participate in conferences, court hearings, meetings and committees as needed; attend other agency meetings such as DFCS Family Team meetings and school meetings as support for participants
REQUIRED KNOWLEDGE/SKILLS/ABILITIES
• Knowledge of principles, best practice standards, procedures, operations and functions of Rockdale County
• Knowledge of applicable county, state and federal laws
• Knowledge of legal terminology, documents, juvenile court procedures, practices and operations
• Knowledge of individual and group behavior, the juvenile justice system, and accountability courts
• Knowledge of counseling techniques, chemical dependency diagnosis and treatment, mental health diagnosis and treatment, and available system and community resources
• Knowledge of the effects of trauma on brain development and youth behavior, and trauma-informed treatment
• Skill in operating modern office equipment.
• Ability to use judgment and discretion
• Ability to establish and maintain effective working relationships with elected and appointed officials, employees and the general public
• Ability to assemble information and generate written reports and documents in a concise, clear and effective manner
• Ability to maintain strict confidentiality
• Ability to handle multiple interruptions and adjustments to priorities daily
• Ability to carry out oral and written instructions and read, interpret and apply laws, policies, rules and procedures
MINIMUM QUALIFICATIONS
• Associate degree in Social Science, Criminal Justice, or a related field; Bachelor’s degree preferred
• Three (3) years working experience in social service or criminal justice or an equivalent combination of education and experience
• Valid State of Georgia driver’s license
