The Care Coordinator - Clinical Services is responsible for assessing client's behavioral health needs and eligibility for services, providing case management support, and coordination of behavioral health services. The Care Coordinator serves as a liaison and advocate for the client to ensure the delivery of quality and timely services, and is responsible for the submission of service plans, assessments, and referrals.

ESSENTIAL RESPONSIBILITIES

  • Screens, evaluates, and schedules referred clients who may be in need of behavioral health services
  • Researches eligibility for behavioral health services
  • Develops and monitors client service plans
  • Provides complete documentation and reporting of client behavioral health services in accordance with agency and funding requirements
  • Attends and participates in all professional meetings and clinical staffing
  • Attends and participates in Child and Family Team and Adult Recovery Team meetings to review progress/barriers and determine appropriateness of services
  • Provides referrals, assists clients in arranging appointments with community resources, and monitors client progress
  • Maintains individual service files for clients that include plans and strategies for achievement of goals and provides follow up contacts to assess progress
  • When indicated in client's services plan, provide support services that include: case management, personal assistance, family support, peer support, advocacy, and transportation services to clients
  • Performs other duties and responsibilities as assigned

WORK HOURS, SCHEDULE & FLEXIBILITY

This position often works weekdays; however there may be some evening or weekend events that will require your attendance in order to best represent the Agency. Some flexibility in scheduling exists, as long as Agency needs are being adequately addressed.

  • Must be able to handle all facets of the business both standard and non-standard situations
  • Ability to work through translation services if not fluent in a second language
  • Must be willing to accept feedback, and respond in professional manner

EDUCATIONAL REQUIREMENTS (must meet one of the following)

  • Master's degree or bachelor's degree in a field related to behavioral health;
  • Is a registered nurse;
  • Is a physician assistant who is not working as a medical practitioner;
  • Has a bachelor's degree and at least one year of full time behavioral health work experience;
  • Has an associate's degree and at least two years of full time behavioral health experience;
  • Has a high school diploma or high school equivalency diploma and a combination of education in a field related to behavioral health and full time behavioral health work experience totaling at least two years;
  • Is licensed as a practical nurse, with at least three years of full time behavioral health work experience ; or
  • Has a high school diploma or high school equivalency diploma with at least four years of full time behavioral health work experience.

  • Bilingual preferred, but not necessary
  • Strong written, verbal, presentation and communication skills
  • Positive attitude, proactive mentality, responsive, and strong work ethic
  • Ability to interact effectively in a team setting
  • Ability to interact professionally and effectively with victims of crimes and trauma
  • Ability to communicate effectively orally and in writing
