In the interest of protecting the health and safety of all patients, associates, and guests, Unity Health has classified some positions as “safety sensitive.” A “safety sensitive” position is any job position in which impaired performance could result in harm to the health and/or safety of self or others. DESCRIPTION:

Responsible for assessment and identification of need for the provision of social work, discharge planning and case management services to the organization’s customers for the purpose of ensuring positive outcomes for customers’ health, safety and well-being. Responsible, jointly with other case management team members, physicians and staff for ongoing post-hospital care planning which is timely and appropriate and which increases customer satisfaction, reduces unnecessary length of inpatient hospitalization and effectively provides for continued quality care. Responsible for the management of the activities of the Social Work Services Department. Accountable for insuring the quality of services rendered within the Social Work Services Department’s scope of services, and accordingly, the planning development and implementation of departmental policies, procedures, programs, services, goals and objectives in keeping with the organization’s philosophy and culture. This position requires knowledge in the care of patients served as indicated: Infant-adolescent is 0 to 17 years, Adult is 18 years to 65 years and Geriatric is 65 years and above.

Job Hazards: Minimal danger of contracting disease if infection control policies and procedures are followed.

Category III: Tasks that involve no exposure to blood, body fluids or tissues. Persons who perform these duties are not called upon as part of their employment to perform or assist in emergency medical care or first aid or to be potentially exposed in some other way.
