The Palliative Care Nurse Navigator will work in collaboration with the Medical Director, Hospice Physicians, and Hospice and Palliative Care Teams, to provide palliative care services to patients and families in a variety of settings. This position is responsible for facilitating the intake of prospective hospice and palliative care patients and will be a partner in educating our staff and the community on palliative care.

This position works collaboratively with the intake team, hospice referral specialists, and the medical team to expedite referral approval. This position reviews records and communicates with hospitals, facilities, clinical offices, and families as needed to gather information. The Palliative Care Nurse Navigator (PCNN)communicates clinical eligibility criteria to the medical team via written summaries. The PCNN has an excellent understanding of local coverage determinations, hospice eligibility requirements, regulations surrounding patients’ admission to hospice care, and the levels of care hospice provides. The PCNN communicates in a sensitive, professional, and empathetic manner, and exhibits good understanding of the hospice philosophy and expertise in the care of the terminally ill.

Essential Functions

  1. Navigator of the program including program structure and goals of palliative care staff in collaboration with the Chief Clinical Officer and Director of Richardson Palliative Care.
  2. Implement the strategy for the RPC team in alignment with the overall SSRCH strategy, to increase growth and representation of people being served.
  3. Oversee day-to-day operations of the Palliative care team including scheduling of visits, stratification of patients, and assisting with goals of care discussions with patients and families.
  4. Mentor staff to successfully implement and execute activities.
  5. Demonstrates expert techniques in clinical and supportive skills to improve referrals to Sharon S. Richardson Community Hospice (SSRCH) and Richardson Palliative Care (RPC).
  6. Participates in SNF, ALF or hospital team meetings with designated referral partners as part of routine quality review meetings.
  7. Ensures that SSRCH’s clinical practice supports the regulatory needs of our referral partners.
  8. Serves as a speaker in outreach activities to educate long-term care and healthcare referral sources, community professionals, influencers, and consumers about SSRCH and RPC.
  9. Assists with the admissions and discharges as needed on selected patients.
  10. Responsible for accurate and timely documentation in CRM and EMR as appropriate.
  11. Knowledge of agency policies and procedures, federal and state guidelines and directives, health care and hospice principles, standards, and best practices.
  12. Maintains a working knowledge of SSRCH’s Emergency Operations Plan and contributes to risk assessment and emergency preparedness efforts.
  13. In conjunction with agency leadership, ensures survey readiness through preparation for internal and external audits.
  14. Utilizes appropriate communication lines to problem solve personal and department issues.
  15. Participates in performance evaluation process for Palliative Care team. Assists with performance improvement plans as needed.
  16. Assists with holidays, after hours, and on call duties as assigned.
  17. All other duties as assigned.

Working Conditions

  1. Physical Requirements: Manual dexterity to operate computer and other office equipment. Able to sit for extended periods of time. Ability to have hearing and vision correctable to normal levels. Ability to locate and drive to and from facilities and to meetings at other locations as necessary. Ability to ambulate on a frequent basis, constant bending, lifting, and twisting.
  2. Mental Requirements: Read, speak, write and understand English; critical thinking skills; ability to add, subtract, multiply, divide and work with fractions; develop plan, procedures, goals, strategies, or processes; create methodologies for accomplishing goals; must be able to function independently without supervision.
  3. Working/Environmental Conditions: Fast-paced changing environment, with multiple directives from several disciplines.
  4. Machines, Equipment, Tools Used: Telephone, computer, copy machine. Will require personal vehicle to travel to various facilities.
  5. Exposures: Possible exposure to blood, other body fluids or other potentially infectious materials. Possible exposure to communicable diseases or infections. Possible exposure to hazardous equipment, assorted chemicals, gases/vapors, and other pollutants.
  6. Environmental Demands: Work is primarily performed in facilities. 80% travel expected.

Professional Qualifications

  1. Certificate/Degree/License Requirements: Registered nurse, CHPN required within 2 years of hire, valid Wisconsin driver’s license upon hire, recertification as required. Must carry car insurance on vehicle used to perform job and an acceptable driving record.
  2. Experience Requirements: Minimum of three years public health, home health nursing or hospice experience. Experience developing and delivering educational offerings to teams (preferred).
  3. Communication Skills: Business acumen with demonstrated ability to understand long-term care, assisted living facilities and medical practice’s function and how to build effective business relationships. Strong public speaking/presentation skills, excellent verbal, and written communication skills. Possess strong problem-solving and critical thinking skills with the ability to analyze data. Maintain working knowledge and awareness of end-of-life issues.
  4. Computer Skills: Proficiency in the use of Microsoft Office (Excel, Access, PowerPoint, and Word) or similar products.
