JOB DESCRIPTION
REPORTS TO: Nurse Manager
PURPOSE:  Assist the medical team with inpatient and outpatient care. Works across all sites.
COMPENSATION: $25-$30/hourResponsibilities and Duties:
  • Assist medical providers with rooming patients, taking vital signs, refilling prescriptions.
  • Request medical records.
  • Assist medical team with inpatient and outpatient care coordination.
  • Review immunizations.
  • Act as liaison with client physicians, including accessing appointments for physicals, transcribing medication orders, communicating with community medical professions on client health needs.
  • Work as member of the clinical team to provide consultation on medical issues presented by potential clients prior to admission.
  • Function as member of the team securing referrals and resources to participants. Attend weekly team meetings.
  • Review health-related policies and procedures specifically related to licensure for Minnesota Department of Human Services Rule 245-G and Supervised Living Facilities and recommend appropriate changes.
  • Review procedures related to prevention of infectious diseases and oversee disposal of medical waste.
  • Enter individual client information on computer.
  • Review and supply first aid kits and standing order medications.
  • Order medical supplies as needed.
  • Provide other services as needed, to be determined by the Medical Director or Nurse Manager.
Job Requirements:
  • Current Minnesota Licensed Practical Nurse licensure.
  • CPR for Healthcare Worker certification required.
  • Minimum of 3 years’ experience as an LPN providing direct services to diverse, underserved populations.
  • Experience in MI/CD or public health preferred.
  • Must have attention to detail, excellent communication skills, proficient with computers, and ability to work well on a team.
  • Must be able to pass DHS background study and meet applicable requirements and regulations.
  • Current Minnesota Driver’s License and good driving record.
  • No DUIs in last three years; No more than three moving violations/accidents in past three years
 
  • Must be 18 years or older.
  • Must be able to lift 25 lbs.
  • Must be able to climb stairs
 WOMEN’S TREATMENT CENTER

Wayside’s Women’s Treatment Center provides residential treatment for up to 40 women at a time and is designed to serve women with co-occurring substance use disorders and mental health diagnoses. Women receive coordinated behavioral/physical health care tailored to their medical needs and treatment history while receiving continuing care and supportive services.

Wayside’s Wellness Outpatient Center provides outpatient, intensive outpatient, and preventative care for women. Our programming meets the needs of women at any stage of their journey—as a step down from residential treatment, before the need for residential treatment to diminish consequences of the disease long-term, and/or those who have received services elsewhere without success.
