Position Overview:

The Case Planner is responsible for all aspects of case management services for a caseload of Hard-to-Place youth residing in the AOBH Program. The case planner provides concrete services, resources, counseling and is an integrated member of the AOBH team which strives to ensure improved outcomes for youth.

Position Qualifications:

  • Masters in Social work, Criminal Justice or equivalent from an accredited college and experience working with adolescents.
  • Must have two years of foster care experience working with adolescents and knowledge of ACS, OCFS and CONNECTIONS.
  • Must understand the critical nature of timely and accurate documentation of service delivery and best-case work practice.
  • Must Knowledge of the Integrated Treatment Model and Dialectical Behavior Therapy
  • Must have knowledge and experience in completing FASP, Permanency Hearing Reports, Transition Plans, and Individual Services Plans
  • Must have knowledge and experience in child development as well as adolescent development.
  • Must be a team player and have the ability to work independently.
