Job DescriptionJob Description

Essential Functions

  1. Ensure family style dining and active treatment is occurring at all times by DSP's in homes. Provide direction to DSP's on how interact with our individuals in order to enhance skill building and independence in areas such as cooking, housekeeping, hygienic practices, and social interaction

  1. Ensure health and safety of individuals by monitoring and participating in activities, supervising appropriate diet and medications, and intervening in crisis situations.

  1. Document all services performed and assure Direct Support Professionals document according to ISP and departmental procedures. Ensure DSP's utilize ISPs to provide designated services at the required frequencies including documenting services provided on each shift and writing incident reports for any unusual activity.

  1. Observe/direct Direct Support Professionals in the implementation of services needed according to the ISP. Orient, supervise, provide ongoing feedback, evaluate and report any disciplinary action needs when necessary.


  1. Routinely observe every staff person and identify training issues. Discuss and implement plans of action for problem solving staffing issues with Home Managers, QIDP's/PD's.
  2. Work in close cooperation with Home Manager, QIDP/PD, nursing staff, other supporting team members, as well as family support in carrying out all daily activities and objectives.

  1. Follow all state, federal and local regulations and Scioto Trails' departmental policies and procedures.

  1. Meet regularly with the Management to discuss issues and concerns.

  1. Be agency representative in county and state reviews.

  1. Ensure that support documents such as work orders, accident/incident reports, and scheduling paperwork are completed as needed.

  1. Responsible for maintaining a safe workplace and complying with all state and federal safety and health regulations as well as company safety policy and procedures.

  1. Perform all other duties as assigned and required.

Minimum Requirements: High School Diploma, Valid drivers license, 1 year of DSP experience
