We're looking for a compassionate and energetic individual to join our Portland team as a full-time Referral Navigator.


Your Role as a Referral Navigator:

You'll be the welcoming face of our agency, providing quality care and support to those who reach out to us. You'll build rapport with individuals from all walks of life, ensuring they feel heard, respected, and connected to the resources that best meet their needs. Your position is office based, although the majority of your interactions will be over the phone.


Key Responsibilities:

  • Intake Coordination: Conduct initial interactions, basic screenings, gather information, and guide individuals through our intake process.
  • Referral Expertise: Become an expert on our programs, services, and community resources, ensuring individuals are connected to the most appropriate level of care.
  • Collaboration: Foster strong relationships with colleagues, external agencies, and community partners to ensure seamless coordination of care.
  • Community Engagement: Represent AWS at community meetings and events to stay informed and strengthen our network.
  • Data Management: Maintain accurate and organized records of referrals, intakes, and outcomes.

Qualifications:

  • Associates degree in social work, psychology, or a related field preferred (not required).
  • MHRTC certification preferred
  • Experience in behavioral health or social services is strongly preferred.
  • Outstanding interpersonal and communication skills to connect with individuals from diverse backgrounds.
  • Strong organizational and time management skills to juggle multiple tasks and priorities.
  • A genuine passion for helping others and making a positive impact.

Requirements:
  • High school diploma or equivalent with 1-3 years of experience in the social service field required. A.S. degree in a related field and MHRT-1 or MHRT-C certification preferred.
  • 1-2 years of experience in a social service program.
  • Essential customer service skills, including empathic listening, needs assessment, program matching, and effective referral to both internal and external services.
  • Proficiency in managing information with accuracy in data entry, verification, and retrieval from various sources.
  • Preferred skills include moderate typing speed with accuracy, proficiency in G-Suite, experience with Electronic Health Records (EHR), and adeptness in using internet tools for research and communication.
  • Ability to multitask across multiple computer programs while engaging with clients via phone or in person.
  • For contracts with Rider D: Must pass criminal background, child protective service, and sex offender checks.
  • Must not be listed on state or federal suspension and debarment lists.
  • Driving may be necessary for meetings and training but is not a primary job duty.
