Schedule: Monday-Friday 9am-5pm.

The PSH Case Manager provides outreach, engagement and advocacy for homeless individuals and families participants in scattered site permanent supportive housing. The Case manager will perform assessment intake, case management and other supportive services to assist persons eligible for housing services and will assist participants in meeting their personal life goals, such as increasing their financial stability, improving their wellness and reaching in greater self-determination. This person should reflect the values of our Faith Statement and mission purpose of Restoring Hope and Changing Lives.

While this position is not a hybrid-remote position, employee may go offsite to conduct house visits, inspections and emergency case management when case management cannot be performed on site.

DUTIES & RESPONSIBILITIES:

Direct Support Responsibilities:

  • Conduct comprehensive assessments and interviews to determine eligibility for the program prior to program entry
  • Provides regular case management and coordination services to PSH participants and families
  • Develop and update client action plans, generated with participant buy in, and advocate for participants so they can obtain services specified in their actions plan
  • Link families to appropriate services and activities within and outside ODM, including employment and training services, legal assistance, substance abuse programs, mental health, schools and other appropriate services.

SKILLS & QUALIFICATIONS:

  • Committed and mature Christian with a commitment to and a passion for "Changing Lives. Restoring Hope" in the lives of the impoverished, addicted and mentally ill.
  • Financial acumen
  • Demonstrated written and oral communication skills
  • Demonstrates strong skills in interviewing, engagements, assessment, planning and advocacy
  • Demonstrates good judgment and ability to work independently
  • Demonstrates experience and ability to interact sensitively and respectfully with persons with disabilities (developmental, physical, mental, substance abuse, etc) and a history of homelessness.

EDUCATION/EXPERIENCE

  • Bachelor's degree in the area of social service
  • Housing experience preferred
  • Knowledge of community resources and work practices with homeless or underserved populations
