In addition, also provides facilitation of CCFCS's emergency shelter telehealth program. This position also provides coverage and support as needed to other programs as needed by the program manager.Completes relevant screenings and assessments including but not limited to psychosocial, mental health, financial status, and employment status.Creates, implements, and monitors comprehensive, person-centered, trauma-informed, and solution-focused plans and goals.
