Director of Nursing, Hospice (We also need a Home Health DON)

Job Summary: The Hospice Director of Nursing is a registered professional nurse who ensures quality of patient care in the home. Primary function is for the overall administration of the clinical departments and monitoring of appropriate staffing and productivity. Coordinates care with the interdisciplinary team, patient/family and referring agency. The position will be the highest level of clinical oversight within the business.

 

General Essential Functions –

  • Directs and coordinates clinical departments and branches; assumes responsibility for continuity, quality, and safety of services delivered in compliance with State and federal regulations.
  • Leads agency IDT meetings and supervises interdisciplinary hospice team including nurses, aides, chaplains,volunteers, etc.
  • Participate in activities relevant to services furnished. including the development of qualifications and assignment of agency personnel
  • Ensure that an appropriate health care professional performs a reassessment of a patient's needs when there is a significant health status change in the patient's condition, at the physician's request or after hospital discharge.
  • Knowledgeable of staffing patterns and work requirements
  • Supervises and provides direction to subordinates, in an effort to ensure quality, compliance with Plan of Care continuity of services by appropriate health care personnel and ensure that a patient's plan of care or care plan is executed as written.
  • Monitors quality of care program and assures appropriate corrective measures are performed.
  • Promotes compliance with all fiscal intermediary and/or other third-party payors, through education, coaching, and other assistance as necessary.
  • Monitors systems and identifies problem areas to Administrator both verbally and through written reports.
  • Meets mandatory continuing education requirements of the agency/licensing board.
  • Promotes and educates regarding the concepts of infection control and standard precautions in

coordinating/performing patient care activities to prevent contamination and transmission of disease.

  • Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
  • The position will interact directly with the company’s ownership and will be the highest level of clinical oversight within the hospice division.
  • Other duties and responsibilities as assigned.
