Job DescriptionJob DescriptionPosition Description: ESSENTIAL DUTIES:
\tModels a service attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workers
\tDemonstrates appropriate skills for initial and follow-up appraisal of patient status. Possesses an adequate knowledge of clinical diseases
\tDemonstrates ability to perform nursing treatments and procedures as prescribed for patients assigned. Administers those treatments in an appropriate manner
\tDemonstrates adequate knowledge of medications commonly prescribed to patients and administers appropriate medications
\tDemonstrates knowledge and awareness of palliative aspects of care and ensures patient safety, comfort and protection
\tEnsures that all residents under his/her direct supervision is provided care and services to attain or maintain the highest practical quality of life.
\tEnsure resident/patient, visitor and co-worker safety by remaining aware of and reporting any new or existing safety hazards
\tMaintain a good attendance record
\tCompetently perform all basic and assigned nursing skills within a licensed vocational nurses scope of practice
\tActively participate in all aspects of patients care, in accordance with current professional standards and practice
\tFollow established standards of nursing practices and implements Faiths policies and procedures to provide quality patient care
\tConducts patient visits/continuous care shifts as schedule by the plan of care, reports problems and communicates appropriate patient information to RN Case Manager
\tAssists with any activities as related to patients and deaths
\tPractices standard infection control techniques
\tNotify the Medical Director, family, administrative staff and direct care providers of patient change in condition and necessary follow up
\tDocuments notes on each patient reflecting nursing care, patient instruction and patient/family response, doctor notification, a specific plan for ongoing care
\tCommunicates unresolved work-related problems using chain of command, consistently demonstrates effective problem-solving skills and handles confidential information appropriately
\tDemonstrates flexibility with a changing workload/caseload sets priorities accordingly and carries out job assignments as directed.
\tConsistently maintains productivity at an acceptable level established by Faith Hospice
\tOther duties as assigned by Patient Care Manager, RN Case Manager or designee Additional Information: MINIMUM QUALIFICATIONS:
\tValid Texas License Vocational Nurse license
\tPrevious acute care, long term care, hospice or home health 1 year experience preferred
\tKnowledge of current professional nursing theory and practice
\tExcellent technical and documentation skills
\tAbility to organize work flow, respond to emergencies and note detail
\tAbility to maintain a positive, stable disposition in performing work in association with staff and residents.
\tAbility to maintain a high level of performance in an environment of frequent demands and changes
\tExcellent verbal and written communication skills
\tAbility to perform Essential Duties as outlined below
\tGenuine caring for and interest in the care of mature adults, handicapped people and hospice Patients
\tBasic computer knowledge required (Internet, word processing, database entry)
\tMust have reliable transportation, maintain auto liability insurance on personal vehicle
\tMust maintain current drivers license and be able to drive to assignments
\tAbility to perform work tasks within the Physical Demand Requirements as outlined below
\tAbility to comply with the Resident/Patient Bill of Rights and Teammate Responsibilities
\tAbility to comply with organizational and departmental safety policies and procedures
