GENERAL SUMMARY

The Wellness Nurse is an RN who provides wellness nursing care to the elderly residents living in a Bethany Community. The Wellness Department provides supportive services, including, but not limited to home care, personal care, homemaking, light housekeeping, medication reminders, laundry/linens, and grocery shopping. This position is a community-based position in an independent housing environment for older adults. The Wellness Nurse will work closely with the Director of Wellness to conduct assessments of residents and to develop individual care plans. The Nurse will also provide direction and support to the team of certified nursing assistants (C.N.A.’s) who provide care to the residents. This wellness position provides emotional and social support to residents and families as they age in place in their community, and they will work closely with residents and families to provide a high-quality way of life by offering services and supports to residents. The Wellness Nurse should have experience with and understanding of an elderly population and the associated programs upon which they rely, such as Medicare and Medicaid, as well as other state and/or federal programs.


ESSENTIAL JOB FUNCTIONS

  • Identify potential residents and conduct initial assessments for eligibility for services.
  • Perform assessments of residents receiving services and update the care plan accordingly. Re-assessments are regularly scheduled and updated.
  • Work closely with the Assistant Director of Wellness regarding care plans and services for residents and ensure appropriate communication to the Director and CNA’s when making any changes.
  • Develop individual care plans for residents based upon the assessment and physician’s recommendations. Interview residents, family members, and care givers as needed to develop an individualized care plan.
  • Educate residents regarding care/services and remain within scope of practice of licensure for the state of Massachusetts.
  • Provide formal case management (including intake) and referral services to all residents needing such assistance (i.e., evaluation of health, psychological and social needs, development of an individually tailored case plan for services and continued ongoing resident assessments) for proper coordination of care/services.
  • Monitor the ongoing provision of services from community agencies and collaborate with associated provider(s)/ agency regarding the resident and their healthcare concerns/services. Manages the provision of supportive services where appropriate under EOEA (Executive Office of Elder Affairs) regulations while remaining under scope of practice.
  • Monitor the health and well-being of residents on a Bethany service program.
  • Be available to meet with all residents regardless of program participation, to discuss health and wellness and to encourage participation in a program when deemed appropriate.
  • Alternate weekend on-call responsibilities with Director of Wellness during off hours to advise CNA’s when needed and to report to work to provide coverage in case of a staff shortage or schedule change.
  • Work seamlessly with other departments (Property Management, Maintenance, Food Service, Activities, etc.) to ensure a coordinated effort of services and high-quality resident satisfaction.
  • Assist the Director with regular audits and reviews, such as the bi-annual Assisted Living recertification.
  • Provide back-up nursing services to all Bethany sites as needed and requested.
  • In coordination with the Director of Wellness provide required orientation and training to all staff who work in the assisted living in accordance as required by EOEA regulations.
  • Follow organizational procedures to collect information.
  • Maintain privacy and confidentiality and responsibility of private personal data, abiding by the organization’s Information Security Policy.
  • Coordinate care with the resident, family member(s), physician(s), insurance companies and other health care providers and relevant parties.
  • This is not an acute care nursing position.

OTHER DUTIES AND RESPONSIBILITIES

  • Attend training and professional development courses and sessions to remain current with the industry, health care programs, and initiatives.
  • Other duties as assigned as may be required to meet the operational needs of the Wellness Department.

Requirements:

COMPETENCIES

  • Knowledge of Federal and State programs (Medicare, Medicaid, MassHealth, Group Adult Foster Care, Senior Care Options (SCO’s), Home and Community Based Waiver Program, Massachusetts State Home Care Program, PACE program)
  • Experience with homecare of community-based software (or web-based) systems is required.
  • Knowledge of and experience with Electronic Health Records (EHR’s) is highly desired.
  • Organizational skills, interpersonal skills, excellent verbal, and written communication skills
  • Experience and ability to work with an elderly population.
  • Intermediate to advanced computer skills are required. Experience with Word, Excel and the internet is required.
  • Long periods of standing may be required. Agility and the ability to push, pull and bend are required. Ability to lift up to 50 pounds unassisted.
  • Ability to read, write and communicate in English is required.
  • Transportation availability and ability to travel between locations is required.


PREPARATION, KNOWLEDGE, SKILLS & ABILITIES

  • A valid and current Registered Nurse (RN) license is required for this position. (LPN would be considered)
  • A valid and current driver’s license is required.
  • A minimum of 2-3 years of nursing experience is required.
  • Experience with an elderly population, the aging process, elder services, disability services, eligibility for and procedures of Federal and applicable State entitlement programs
  • Demonstrated working knowledge of supportive services and other resources for senior citizens and non-elderly disabled.
  • Demonstrated ability to advocate, organize, problem solve and provide results for the elderly and non-elderly disabled.
