*Job Summary:*
We are seeking a compassionate and experienced Physical Therapist to join our Home Health Care team. As a Home Health Care Physical Therapist, you will provide high-quality, patient-centered care to patients in their homes, focusing on improving mobility, strength, and function.
*Responsibilities:*
- Conduct initial and ongoing assessments of patients' physical therapy needs- Develop and implement individualized treatment plans- Provide skilled physical therapy interventions, including exercises, manual therapy, and modalities- Educate patients and caregivers on proper techniques and strategies- Monitor patient progress and adjust treatment plans as needed- Collaborate with interdisciplinary teams, including physicians, nurses, and occupational therapists- Document patient information accurately and timely- Participate in case conferences and quality improvement initiatives
*Requirements:*
- Bachelor's or Master's degree in Physical Therapy- Current PT licensure in the state of Florida- Minimum 1-2 years of experience in physical therapy, preferably in home health care- Strong clinical skills, critical thinking, and problem-solving abilities- Excellent communication, interpersonal, and patient education skills- Ability to work independently and as part of a team- Reliable transportation and ability to travel to patient homes
*Preferred Qualifications:*
- Home health care experience or certification (e.g., HHC PT, OASIS)- Knowledge of Medicare regulations and guidelines- Experience with electronic medical records (EMRs) and mobile documentation systems
