Overview

The Scheduling Social Work position is responsible for the administrative oversight and supervision of direct reports at assigned site that will include master’s level social workers, as well as other team members as needed. Promotes effective collaboration and care planning amongst interdisciplinary team members. Responsibilities include, but are not limited to: coordination of routine assessments, care planning, and other related tasks within the scope of practice; assisting with family conferences, community referrals, auditing team member documentation, onboarding and orientation of new hires as well as routine competency and work performance evaluations. Utilize knowledge of state and federal regulatory requirements as they relate to social work department tasks, in order to provide effective oversight and ensure compliance.  Collaboration with other site leaders to monitor site safety, overall staffing changes, and program budgets.

 

Other Responsibilities

1. Provides leadership to Social Workers I & II in the performance of initial and semi-annual assessments, quarterly notes and develops care plans for assigned team/participants of Senior Care Services Program and psychotherapeutic interventions of participants and assure completion within monthly timeframe set by state guidelines.
2. Mandatory attendance and active participation in all program team proceedings.

3. Provide leadership/support to Social Workers in the development and implementation of individual’s plan of care.

4. Provide oversight and direction for Social Workers to advocate for their participants in family conferences as needed to resolve difficult issues.

5. Active participation in Senior Services process of improvement team efforts and all quality assurance improvements.

6. Perform group and individual counseling and therapeutic support to program participants.

7. Provide training support and case oversight in coordination with Social Work team.

8. Act as a liaison with participant’s family, caregiver and for with other community agencies providing services to program participants.

9. Document all participant and family contacts in accordance with program guidelines ensuring that the documentation of all contacts in accordance with standards and regulations.

10. Interview and screen senior care services participants, family members and caregivers to determine additional psychotherapeutic interventions.

11. Make home visits to participants as necessary.

12. Mandatory contacts with participants within the assigned caseload at the least one time a month.

13. Perform other related duties as assigned.


This position works Monday through Friday 8a-5p onsite.
