JOB SUMMARY:
The Clinic Care Coordinator serves patients during and between primary care clinic visits, to help patients manage their own conditions through education, lifestyle change and teaching self-management skills. This position is key to ensuring safe and effective movement of patients across the care continuum, serving as the liaison between the professional staff in a care setting and the patient and/or family. In this role the coordinator provides information and guidance to the patient and/or family for effective care and enhanced patient-practitioner communication.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Uphold and support the philosophies, objectives and policies of CHSI.
2. Identify and utilize cultural and community resources; link families to public benefits.
3. Establish and maintain relationships with identified service providers.
4. Arrange special training events.
5. Prioritizes disease management and activities according to intensity, need, and required follow-up.
6. Serves as a guide to patient, coaching the patient in addressing chronic disease management.
7. Evaluates aspects of each patient’s condition, diagnoses, medications and support systems to formulate and individualize plan which will lead to successful outcomes in:
a. Medication self-management
b. Appropriate primary care and specialist follow-up
8. Promotes patient self-management and empowers patients/families to achieve maximum levels of wellness and independence.
9. Collects and analyzes data on clinical indicators to identify opportunities for improvement. Develops, prepares and presents related records and reports. Participates in measuring clinical outcomes, data procurement and analysis activities.
10. Maintains appropriate professional boundaries.
11. Maintains a working knowledge of, and adheres to applicable federal/state regulations including, but not limited to, laws related to patient confidentiality, release of information, and HIPAA.
12. Uses safe work practices. Promptly reports workplace and patient safety issues to supervisor.
SUPERVISORY RESPONSIBILITIES:
This position does not require supervisory responsibilities.
KNOWLEDGE, SKILLS AND ABILITIES: Must have a strong interest to work collaboratively with chronic disease patients to help them manage their condition Ability to maintain patient confidentiality Excellent written, verbal and listening communication abilities Willingness to establish effective working relationships with internal and external customers Ability to manage conflict, stress and multiple simultaneous work demands in an effective, professional manner. Decisions and problem solving require a combination of analysis, evaluation, and interpretive thinking Knowledge and appreciation of cultural diversity and ethnic background Ability to work with low-income adults and families Computer literacy, including but not limited to data entry, retrieval, and report generation Ability to work with patients/families of all ages in a variety of settings, physical conditions and social/cultural environments Ability to drive
EXPERIENCE AND EDUCATION: MA or LVN Must have a valid California Driver License Minimum of three years of experience in clinic setting preferred Experience with chronic disease management such as comprehensive diabetes, asthma and/or COPD care
WORKING CONDITIONS:
Normal office environment; office, clinical or patient care setting to include physician offices, clinics and outside community events; may be subject to adverse weather conditions, slippery surfaces, and extreme temperatures.
Physical requirements include: Climbing: Ascending or descending stairs, ramps, etc. Body agility is emphasized. Pulling: Using upper extremities to exert force in order to drag; haul or tug objects. Fingering: Picking, pinching, typing or otherwise working, primarily with fingers rather than with the whole hand or arm. Talking: Expressing or exchanging ideas by means of speaking; conveying detailed or important information. Hearing: Ability to receive detailed information. Visual: Requires extensive writing, computer terminal and reading. Repetitive Motion: Substantial movements of wrists; hand and/or fingers. Sitting: Must be able to sit and perform data entry for at least 8 hours a day. Dexterity: Activities require using fine motor coordination.
