Duties and Responsibilities

Intake Coordination:

·        Provide front desk coverage three evenings per week to provide support to clinicians and to field any incoming intake or resource calls

·        Receive all new referral calls/requests and return messages in a timely fashion

·        Verify insurance for all new referrals and on a monthly basis for those on waitlist

·        Follow up with those on waitlist regularly (i.e. monthly) to assess continued need and interest in services

·        Complete all initial screenings including trauma assessment, suicidal ideation and health risks

·        Maintain consistent communication with Program Director and Clinical Supervisor to ensure all new referrals are processed appropriately or referred to appropriate community resources

Resource Coordination:

·        Receive referrals from outpatient therapists and other agency departments to assist with resources for program participants related to housing, food, clothing, transportation, benefits and other needs as requested

·        Maintain community resource and agency referral list

·        Conduct follow up with participants and their support systems as appropriate and needed

Counseling:

·        Develop a service plan in collaboration with participants.

·        Develops and maintains therapeutic alliance while maintaining professional boundaries.

·        Complete all necessary documentation in participant’s records within 48 hours of each event.

·        Complies with all agency policies and procedures

·        Maintains HIPAA standards

·        Complete 15 hours of training annually.

·        Attend monthly team meetings

·        Meets with Clinical Supervisor for supervision as scheduled

·        Represents the agency in a positive manner at all times.

·        Performs other duties as requested or assigned, verbally or in writing by supervisor.

Qualifications

·        Master’s degree in counseling, psychology, social work, or related human service field.

·        Solid knowledge of assessment and counseling practices and principles.

·        Knowledge of community resources in Delaware County and mental health services in general.

·        Ability to work independently and as a part of an interdisciplinary team.

·        Proficient computer skills with Microsoft Office applications (Excel, Word, Power Point).
