Position Summary:

We are seeking a highly skilled and compassionate Nurse Practitioner to join our provider network. In this role, you will be responsible for conducting in-home wellness risk adjustment assessments for Medicare and other populations. Your primary objective will be to assess the overall health and well-being of member beneficiaries to ensure accurate and comprehensive risk adjustment coding, leading to greater value-based care.

Nurse Practitioner Details:

  • Location: Combination of remote work and home visits throughout the Pierce County, WA area
  • Schedule: Flexible scheduling. Part-time and full-time opportunities available
  • Both 1099 Independent contract

Nurse Practitioner Compensation

  • The pay for this position is $110 per assessment
  • Benefits are available to full-time employees after 90 days of employment
  • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Nurse Practitioner Qualifications:

  • Active NP license in the state of Residency
  • Care/Case Management experience required
  • Must have solid computer and documentation skills

Nurse Practitioner Responsibilities:

· Perform annual wellness visits and health assessments on a population with chronic conditions.

· Deliver patient health education opportunities.

· Assist in closing quality care gaps (i.e. screenings and labs)

· An enthusiastic collaborator contributing to the enhancement of care delivery.

Nurse Practitioner Preferred Qualifications:

· Must have a valid unencumbered NP License for the state you will be working in

· Open to new graduates and experienced NPs

· Previous In-home Risk Assessment experience a plus but not required.

· 3 years patient care experience highly preferred. (Experience in primary care/adult/geriatric, EMR skills preferred)

· Bilingual a plus
