Job DescriptionJob Description

We are seeking a Speech Language Pathologist to join our home health team You will evaluate and treat patients with speech, language, voice, and fluency disorders. 

Responsibilities:

  • Implement standardized tests for accurate diagnosis of communication disorders
  • Establish treatment goals and milestones with patients
  • Provides services in speech pathology in accordance with the physician’s plan of treatment. 
  • Records and reports client’s reaction to treatment and any changes in the client’s condition to the physician and other members of the interdisciplinary team
  • Instructs and advises client and family of the phases of speech pathology in which they may assist the client
  • Assesses the client’s communication, oral-motor integrity, cognitive status, memory, and problem-solving activities, hearing and speech fluency and quality
  • Evaluates, and regularly reevaluates, the speech pathology needs of the client 
  • Initiates, develops, implements, and makes necessary revisions of the client’s plan of care
  • Conducts OASIS assessments as appropriate

Qualifications:

  • Familiarity with communication disorders
  • Ability to build rapport with patients
  • Excellent written and verbal communication skills
  • Positive and patient demeanor

Requirements:

  • Licensed in the state as a Speech Language Pathologist
  • Minimum of 6 months of experience in Speech Pathology
